The Empire Plan covers Urolift procedures over age 45.  For men younger than age 45 with symptoms of BPH<\/a><\/strong>, insurance pre-authorization from UnitedHealthcare (The Empire Plan) will be necessary to establish insurance coverage.<\/p>\n
