What is Aquablation Therapy for Treatment of BPH \u2013 WaterJet Ablation Therapy for Enlarged Prostate<\/strong><\/h2>\n

\"\"Aquablation<\/strong> refers to a new approach to removing obstructive prostate tissue through the use of high-velocity saline (saline is essentially water with some minerals).   Aquablation is a surgical procedure designed to replicate the efficacy of TURP or prostate enucleation<\/a><\/strong> using waterjet ablation therapy.    <\/p>\n

How is Aquablation for Treatment of Enlarged Prostate Performed?<\/strong><\/h2>\n

Aquablation <\/strong>is performed by a urologist in the operating room under general anesthesia.  The procedure is performed with ultrasound guidance by performing a transrectal ultrasound of the prostate.  Transrectal ultrasound maps the prostate anatomy<\/strong> which is then fed into the AquaBeam surgical robotic planning system.  The urologist decides<\/strong> which area of the prostate to treat and programs the Aquablation robotic system<\/strong> to ablate the particular part of the prostate that needs treatment.  Cystoscopy is performed and the Aquablation delivery device is positioned within the prostate through the cystoscope. Robotically guided delivery of high velocity heated saline according to the planned program chosen by the urologist.<\/p>\n

How Does Aquablation Compare to Laser Enucleation of Prostate?<\/strong><\/h2>\n

There have been no direct studies<\/strong> comparing Aquablation using the AquaBeam system to laser enucleation of the prostate (HOLEP).  It appears that Aquablation<\/strong> results in similar outcomes as TURP<\/a><\/strong>.  Based on indirect comparisons, most studies suggest that prostate enucleation<\/a><\/strong>, especially for large prostates, result in superior long-term outcomes compared to TURP.  It is likely that AquaBeam Robotically guided waterjet ablation therapy for BPH using Aquablation achieves similar outcomes to laser enucleation of the prostate.<\/p>\n

What are the Risks of Aquablation?<\/h2>\n

Post-Aquablation it is difficult to pinpoint the source of bleeding and cauterization can sometimes be challenging.  All patients require an inpatient hospital stay<\/strong> with continuous bladder irrigation (CBI) or balloon tamponade of the prostatic area to stop bleeding<\/strong>.  This is also a risk with TURP but with modern TURP methods with direct visual control of bleeding during the procedure it is rare to require balloon tamponade of the prostate.<\/p>\n

What are the Advantages of Aquablation?<\/strong><\/h2>\n

ProCept Aquablation AquaBeam System does not generate much heat<\/strong> which limits potential injury to periprostatic nerves that can occur with traditional TURP procedure.  Heat injury is a suspected cause of occasional erection problems<\/a><\/strong> in men after TURP.   There is probably no significant advantage of Aquablation compared to bi-polar TURP<\/a><\/strong> and laser prostate enucleation in terms of heat degeneration.  Prostate enucleation using Holmium laser<\/a><\/strong>, for example, is very precise in terms of heat generation, and heat is delivered to the area of less than a millimeter at a time, which is under the surgeon\u2019s control.<\/p>\n

Aquablation<\/strong> is fast with the surgical time of fewer than 5 minutes however the total time that is required to perform the procedure including ultrasound generation is similar to TURP with about 30-35 minutes based on the results of the Aqua study, a randomized controlled study comparing Aquablation using an Aquabeam surgical planning system to traditional TURP.<\/p>\n

Aquablation<\/strong> results in improved quality of life<\/strong>, improved urine flow<\/strong>, and improved urinary frequency and urgency symptoms, similar to what is achieved with TURP.<\/p>\n

The Water Study, a randomized control trial of Aquablation and TURP showed that urinary symptoms significantly decrease (lower is better) from 23 to 6 (improvement of 70%<\/strong>).   Urine flow is also significantly improved with the maximum flow of 21 ml per second from 6 (higher is better, stronger urine stream<\/strong>).   Similarly, residual urine volume<\/strong> decreases from 97 ml to 50 ml with both TURP and Aquablation, a good improvement.<\/p>\n

Aquablation<\/strong> seems to preserve ejaculatory function in most men. <\/p>\n

What are the Disadvantages of Aquablation?<\/h2>\n

Aquablation is associated with a risk of bleeding<\/strong>. As a result, the procedure always requires an overnight hospital stay. In comparison, men can go home after procedures such as Rezum procedure<\/a><\/strong> and Urolift procedures<\/a><\/strong>. Many if not most men are able to go home the same day after prostate enucleation (HOLEP<\/a>), Greenlight laser<\/a><\/strong>, and bipolar TURP.<\/p>\n

Because Aquablation requires an overnight hospital stay, it is more expensive<\/strong> than alternative treatments.<\/p>\n
