What is a Median Lobe of the Prostate and How Does it Matter?<\/h2>\n

\"\"The prostate gland in men<\/strong> has a few distinct components. The lateral lobes of the prostate,<\/strong> which tend to be the largest, are the most common cause of enlarged prostate symptoms in men<\/a>.  Oral pills<\/a> usually provide significant benefits to men with enlarged lateral lobes of the prostate.<\/p>\n

The median lobe of the prostate<\/strong> is another part of the prostate that enlarges in some men. It can also cause blockage of the urinary flow.  The median lobe of the prostate tends to cause more severe symptoms<\/strong>. Oral medications<\/strong> such as Tamsulosin (Flomax)<\/a>, Alfuzosin<\/a>, and Rapaflo<\/a> do not<\/strong> help men with a significantly enlarged median lobe of the prostate.<\/p>\n

A large median lobe of the prostate acting as Ball-Valve<\/span> is a common reason why pills for enlarged prostate such as Flomax<\/a> and finasteride (Proscar)<\/a>, silodosin (Rapaflo)<\/a>, and Uroxatral (alfuzosin)<\/a> do not work well for some men. <\/strong><\/p>\n

Symptoms of BPH but Medications Do Not Help<\/h2>\n

A commonly encountered situation is that men improve a little bit<\/strong> on medical therapy (Flomax<\/a>, Uroxatral<\/a>, Avodart, Proscar<\/a>, Rapaflo) but not as much as an experienced urologist would expect.  When medications for enlarged prostate do not work<\/a>,<\/strong> further evaluation is initiated.<\/p>\n

Symptoms of a Large Median Lobe of the Prostate<\/h2>\n

Men with a prominently enlarged median lobe of the prostate often experience weak urine flow<\/a><\/strong>, frequent urination<\/a><\/strong>, urinary urgency, or may experience urinary retention<\/a><\/strong>.<\/p>\n

Quite commonly, in this situation, the patient may have a very large prostate,<\/strong> which is beyond the effectiveness of medical therapy.  Another common situation is a prominent, large median lobe of the prostate protruding into the bladder<\/strong> and blocking the urine flow in a ball-valve fashion<\/strong>.<\/p>\n

Timely Decision that Medications Cannot Help is Important For Men with a Large Median Lobe<\/h2>\n

Medical therapy is usually not effective for men with large obstructive median lobes. <\/strong> In this situation, minimally invasive surgical therapy with prostate enucleation (HOLEP)<\/a><\/strong>, a Greenlight laser of the prostate<\/a><\/strong>, or bipolar TURP<\/a><\/strong> is usually necessary to provide adequate relief of symptoms and long-term effectiveness. <\/p>\n

The Rezum procedure<\/a><\/strong> can help many men with a small and medium-sized median lobe of the prostate.  Smaller median lobes<\/strong> can be treated with a Urolift procedure<\/a><\/strong>.<\/p>\n

While many patients are hesitant to commit to surgical treatment, sooner or later it becomes necessary in a situation where a large median lobe obstructs urine flow<\/strong>.  The most common regret that patients experience in this situation is that they did not have the procedure done sooner<\/strong>.<\/p>\n

While medications are a good treatment option<\/a> when they are effective, surgical therapy should be offered promptly to men who do not benefit from medical therapy<\/strong> after a thorough evaluation to exclude other causes of poor response to medications for BPH such as a coexistent overactive bladder<\/a>, nocturia caused by low DDAVP, bladder cancer<\/a>, and other causes.<\/p>\n
