Phimosis Symptoms
Some men are unable to pull back the foreskin at all. For some men, it may be a new problem. Some men are born with phimosis and have experienced it all their adult life.
Other men have a milder form of phimosis. Sometimes phimosis may not be evident when the penis is not erect. For men with mild phimosis, difficulty with pulling back the foreskin only occurs when the penis is fully erect.
Symptoms Caused by Phimosis in Adults
Some patients have pain and difficulty retracting the foreskin only when the penis is erect. This causes difficulty with sex.
Phimosis can also cause urinary infections and difficulty with urination. Pain during foreskin retraction can also be caused by the short frenulum.
Treatment Options for Phimosis
The most reliable treatment for phimosis is circumcision. At New York Urology Specialists, we offer same-day circumcision. A dorsal slit is usually performed in urgent or emergent situations. Compared to circumcision, dorsal slit provides a less appealing aesthetic look. For men who have fibrosis (scarring or thickening of the foreskin) especially when it’s present at the level of the corona of the glans penis, the dorsal slit may not provide an adequate outcome for sexual intercourse. In some circumstances, corticosteroid creams can be used for the treatment of mild phimosis. This treatment is more effective in children.
In adults, phimosis is often associated with scarring and fibrosis which often does not respond well to corticosteroid cream or recurs. If a patient wants to try corticosteroid cream, usually it is prescribed for 2-3 months to determine if it works.

How Effective is Treatment for Phimosis?
The most effective treatment for phimosis is circumcision. At New York Urology Specialists we offer a choice of in-office circumcision or circumcision in an outpatient surgical center or a hospital.
For mild phimosis, especially in children, a corticosteroid cream may be effective when applied over a period over a few months. In adults, mild phimosis can be treated with steroid cream but if phimosis is severe and of long duration, the recurrence rate is high and the success rate is <30%.
Treatment for phimosis has a very high success rate. Circumcision is more than 95% effective in the treatment of phimosis. Steroid creams are less effective with a success rate of up to 30% in adults.
How Satisfied Are Patients After Treatment of Phimosis?
Patients who undergo circumcision for treatment of phimosis are very satisfied. The greatest regret of men who underwent circumcision for treatment of phimosis is that they did not undergo it sooner.
