Unable to Pull Back Foreskin. Scarring of the Foreskin.
RG is a 49-year-old man who is visiting New York from another state to address his health concerns. He has been unable to retract the foreskin for the past 5-6 years. He saw multiple urologists and was diagnosed with balanitis xerotica obliterans (BXO). He was treated with steroid creams with no significant improvement in symptoms.
After a discussion of treatment options, the patient decided to proceed with circumcision with the understanding that his recovery will be prolonged (a few months) while the area heals. He was advised that he may require a skin graft but 80-90% of the time the area heals well without a graft.
He underwent circumcision with glansuloplasty with self-absorbable (dissolving) stitches 1 month after his initial consultation. The procedure was done under local anesthesia and took just over 2.5 hours due to severe scarring. The patient was found to severe fibrosis and scarring. He felt minimal discomfort with a pain score of 2 out of 10 during the procedure.
On follow-up 2 weeks after the procedure via the patient portal, the patient reported increased sensitivity of the glans. He was advised that the sensitivity typically resolves within 1-2 months after the procedure.
On follow up via a video-consultation 1 month later, he reports that he has mild sensitivity. A review of the images that the patient uploaded to the patient portal revealed healing proceeding as expected. The patient had additional communications with the doctor via the patient portal on a regular basis.
3 months after circumcision, the patient uploaded updated images after circumcision; the area was noted to have healed well. He was advised that he can resume sexual activity. The patient was highly satisfied with the outcome of the procedure.
