What is Sleeve Circumcision?

Sleeve circumcision refers to a surgical technique of performing circumcision. In sleeve circumcision, the surgeon marks two parallels lines circumferentially where incisions are made.  The foreskin between these lines is removed.  The edges of the two circumcision lines are sutured together to reapproximate the skin edges.

Sleeve circumcision is the most common method of circumcision in adults in the United States.  It is also the most common method of circumcision for children other than newborns. 

Sleeve circumcision can be used to treat foreskin problems including recurrent balanitis, phimosis, frenulum breve, BXO, penile cancer and to perform cosmetic circumcision. The sleeve circumcision technique is also used for circumcision revision.

What are the Advantages of Sleeve Circumcision

While in many situations, sleeve circumcision and clamp circumcision result in similar results when performed by highly skilled surgeons, each has certain advantages.

Sleeve circumcision allows greater versatility and can be used in any situation, including in patients with severe phimosis, BXO (balanitis xerotica obliterans), and frenulum breve.  Clamp circumcision is more limited and cannot be used in the treatment of more complex situations such as BXO, severe fibrosis, or edema.  Clamp circumcision is typically easier to perform and requires less training.

Traditionally, sleeve circumcision allows for more precision in the location of the incision.  However, our modification to the ShangRing circumcision procedure allows us to achieve similar precision for many men.

What are the Alternatives to Sleeve Circumcision?

Sleeve circumcision is a common technique of circumcision. Clamp circumcision is another circumcision technique. Clamp circumcision is the most common method of circumcision in newborn boys in the US. 

There are a number of different clamps used for circumcision. At New York Urology Specialists we use ShangRing, which is an innovative circumcision clamp.  Gomco clamp and PlastiBell clamps are commonly used for circumcision in newborns and babies.  In the Jewish tradition, mohels use a different technique that is similar to the ‘sleeve circumcision” utilizing a Mogen Circumcision clamp. 

A new clamp-based technique CircCurer which is an automatic stapled anastomosis combines the benefits of sleeve circumcision and clamp circumcision. It is going to be available in the US in a few years.

A dorsal slit is sometimes mistakingly classified as a circumcision technique.  Dorsal slit does not result in ‘circumcision’ and as a result, it is not a circumcision technique.  A dorsal slit can be performed alone for the treatment of phimosis or can be combined with sleeve circumcision or clamp circumcision (for example ShangRing circumcision). 

How Long does Sleeve Circumcision take to Perform?

Typically sleeve circumcision in adults takes about an hour to perform. In children, sleeve circumcision can be performed in less than 30 minutes.

Anesthesia, Pain, and Recovery after Sleeve Circumcision

Sleeve circumcision can be performed under local anesthesia or under general anesthesia.  At New York Urology Specialists we usually perform sleeve circumcision under local anesthesia.

What is the Relationship between Sleeve Circumcision and “Circumcision Style”. 

Circumcision style refers to the location of the scar after circumcision.  Sleeve circumcision technique can be used to achieve any circumcision style: low-cut or high-cut, tight or loose.  Sleeve circumcision refers to the technical details of how circumcision is performed, not the circumcision style.
