What is Phimosis?

Phimosis refers to the uncircumcised penis, where the foreskin cannot be pulled back over the head of the penis.  Normally when the penis is uncircumcised, the foreskin can easily be pulled back for cleaning as well as during sex when putting condoms on.

When the foreskin cannot be pulled back, men have difficulty with sex, sometimes experience pain during sex, may experience small cuts and bleeding.  Men may develop swelling or infection of the foreskin.  It may also cause hygienic problems and infection (balanitis or inflammation of the foreskin of the penis) or UTI (urinary tract infection).

If phimosis is less severe, skin tightness, pain, and inability to pull back foreskin may only be present when the penis is erect.  When phimosis is more severe, the foreskin may be so tight that it cannot be pulled back when the penis is not erect.

What is the Treatment for Phimosis?

In general treatment for phimosis is either medical with a steroid cream or surgical with circumcision.  The most appropriate choice of treatment depends on the patient’s age as well as the severity of phimosis.  At New York Urology Specialists we offer same-day circumcision; most men are able to go back to work the next day.  Our circumcision prices are affordable with or without insurance.

Creams for Treatment of Phimosis

Treatment with corticosteroid cream such as 0.05% betamethasone works about 40% of the time in children.

In adults corticosteroid cream is usually effective if the problem (phimosis) is new – has been present for less than 2-3 months and mild.  In adult men who have severe phimosis, corticosteroid cream is usually not effective. Men who have had phimosis all their adult life or more than a few months, corticosteroid cream is typically ineffective – it may improve symptoms temporarily while the cream is applied but phimosis typically recurs.   

In patients who have fungal balanitis (infection) causing phimosis, a combination of an antifungal cream such as 1% clotrimazole and betamethasone often helps if the problem is mild and new. When the problem keeps recurring or phimosis is severe, circumcision is recommended.  In men with diabetes, fungal balanitis tends to recur often, and circumcision is typically the preferred treatment option.

Circumcision is the most effective treatment option for phimosis with a greater than 95% success rate in curing phimosis.  Circumcision has a similarly high success rate in curing balanitis.

Treatment of Phimosis in Adults

In adults, phimosis tends to be more severe with more pronounced scarring of the foreskin.  While corticosteroid creams may be useful for some adults with less severe phimosis, men with more severe phimosis and with long-standing phimosis typically do not benefit from steroid cream or phimosis recurs after the cream is stopped.

In general, there is no downside in trying steroid cream except for the loss of time.  Typically, betamethasone cream or equivalent is used.  It is important to see an experienced urologist if the problem persists more than a few weeks as phimosis and balanitis may also be caused by penile cancer.

Circumcision is the most definitive treatment for phimosis in adults with a very low risk of recurrence of less than 5%.

In adults, freestyle sleeve circumcision is usually performed.  When phimosis is severe, dorsal slit followed by sleeve circumcision is performed.

What is Redundant Foreskin?

Redundant prepuce or redundant foreskin is not the same as phimosis.  Redundant prepuce or redundant foreskin refers to a normal uncircumcised penis.

Partial Circumcision or Dorsal Slit procedure

Dorsal slit procedure is usually performed in emergency situations.  The risks and recovery after dorsal slit procedure and circumcision are fairly similar.  Dorsal slit procedure results in suboptimal aesthetic outcomes.  Partial circumcision can be performed to leave some redundant foreskin but there is no advantage to it.  It may also cause functional problems.  For these reasons, full circumcision is usually performed.

Treatment of Phimosis in Children

Phimosis in very young children is a normal finding and typically resolves with age.  No treatment is necessary unless it causes problems such as infections, poor hygiene, or pain with urination.

In older children if phimosis does not resolve spontaneously, or if there is no obvious phimotic band (scar tissue), treatment with a corticosteroid cream is often effective.  If steroid cream does not help resolve phimosis, circumcision is an appropriate treatment option.

What is the Cause of Phimosis in Adults

Phimosis is usually caused by abnormal scarring of the foreskin usually caused by trauma, which may be microscopic and unrecognized. Microtrauma is very common during sex but usually heals without a problem. When it heals with a scar, phimosis may occur. Infections may also predispose to phimosis.  In men who have lifelong phimosis, the cause is not readily identifiable.

Phimosis Stretching Exercises – Do They Work?

Phimosis stretching exercises may be beneficial for some children and adult men with mild phimosis in combination with steroid cream.  For more severe phimosis, stretching typically does not help and may often lead to more severe scarring and more pronounced phimosis.  Foreskin stretching is usually not effective in adults, as the problem tends to recur.

Phimosis and Frenulum Breve

Phimosis often coexists with frenulum breve, which is a short frenulum of the penis, a tissue band on the underside of the penis that sometimes causes pain with erections.  When phimosis co-exists with frenulum breve, circumcision alongside with frenuloplasty addresses both problems in one procedure.
