AMS 700 Penile Prosthesis: State of the Art in Penile Implant Surgery

The AMS-700 three-piece penile prosthesis from Boston Scientific is one of the two inflatable penile implants available in the US. This 3-piece inflatable penile prosthesis includes several features aimed at greater patient satisfaction.

Men who suffer from erectile dysfunction (impotence) can benefit from the AMS-700 inflatable penile prosthesis. The implant closely mimics natural erections. It provides a firm, rigid erection for satisfactory sexual intercourse and penetration when inflated. The penis returns to its natural, flaccid or non-erect state when the device is deflated.

How Does the AMS-700 Penile Implant Work?

During the insertion procedure for the AMS-700 three-piece penile prosthesis, a pair of cylinders are implanted in the penis, a pump is placed inside the scrotum, and a reservoir containing sterile saline solution is placed in the lower abdomen. Pumping (squeezing and releasing) the pump in the scrotum (“ballsac”) fills the cylinders in the penis with the saline solution (sterile water) from the reservoir and creates a natural-looking erection.

When the deflate button is pressed, the saline is returned to the reservoir and the penis returns to its natural, flaccid (non-erect) state.

Most men find the AMS-700 inflatable penile implant easy to use. The device features a one-touch button for deflation as well as a lock-out valve that reduces the chances of auto-inflation. Once the device is activated, the erection can be maintained for as long as desired.  It is important to deflate the prosthesis after use to prolong its useful life and minimize the risks of complications.

Penile Prosthesis Insertion Procedure: What to Expect?

The implant procedure is usually done on an outpatient basis. All the device components are inserted through a small incision which can be either penoscrotal (near the penis and scrotum) or infrapubic (near the pubic bone).

At New York Urology Specialists, we prefer the penoscrotal approach due to a lower risk of nerve damage. The AMS penile implant comes with or without pre-connected cylinders and pump, giving the surgeon more choices and the ability to tailor the procedure to a patient’s specific needs.

The surgery is performed under anesthesia (general or spinal). It is not uncommon for men to experience some pain and soreness at the surgical site for a few days to a few weeks. Patients are able to return to light routine activities within a few days. We typically advise men to wait 6 weeks before sexual intercourse.

What Are the Risks of Inflatable Penile Implant Insertion?

As with all surgeries, there is a small risk of complications. The risks associated with modern penile prosthesis insertion procedures are small. However, potential complications may include:

• Infection (this may require removal of the implant)
• Pain (especially during the healing process)
• Mechanical failure of the implant requiring replacement

Benefits of AMS-700 Penile Implant

There are numerous benefits of the AMS-700 penile implant. The AMS-700 penile implant is one of two inflatable penile implants available in the United States, the other one being the Coloplast Titan penile prosthesis.

The AMS-700 penile prosthesis has been used successfully in thousands of men over the past 20 years. Patients who are implanted with the AMS-700 penile implant report high satisfaction rates, as do their partners.

The AMS-700 penile implant is available in multiple lengths and in three versions: AMS-700 LGX, AMS-700 CX, and AMS-700 CXR. The variety of choices and options allows the surgeon to choose the most appropriate penile implant for the patient.

AMS-700 penile implant is covered by InhibiZone, which is an antibiotic sheath that covers the penile implant and minimizes the risk of infection. With modern antibiotic prophylaxis, the infection risk with AMS-700 penile implantation is around 1 to 2 percent but can be higher in patients with poorly-controlled diabetes, renal insufficiency, and other risk factors.

The AMS-700 is coated with parylene to enhance its long-term durability and reliability as a permanent treatment option for ED.

AMS-700 LGX Penile Implant with Controlled Expansion

The AMS-700 LGX penile implant model allows for controlled expansion of girth and length of the implant for up to 25 percent after implantation, which may allow patients with penile shortening after radical prostatectomy or as a result of erectile dysfunction to regain some of their lost length over time. For this reason, we advise all our patients to do daily cycling of the penile implant by inflating it for 5 minutes twice a day for 1 year after implantation.

AMS-700 CXR

For patients with severe fibrosis and scarring from prior surgery or Peyronie’s disease or after priapism, an AMS-700 CXR penile implant is available, which may be easier to implant in patients with a lot of scarring of the penis. 

AMS-700 CX

The AMS-700 CX penile prosthesis provides more rigidity and is preferred for use in patients with Peyronie’s disease who undergo penile implant with intraoperative penile modeling, plaque excision with grafting, or penile plication.

Mechanical Reliability of AMS-700 Penile Implant

The AMS-700 penile implant has been used for more than 15 years with excellent results. The rates of mechanical failure have come down significantly since prior models of penile implants used in the 1980s and early 1990s.

Most patients with an AMS-700 penile implant can expect to have it for more than 15 years without the need for revision.

MRI Compatibility of AMS-700 Penile Implant

All components of the AMS-700 penile implant are MRI compatible up to 3-Tesla, which means that it is compatible with all modern MRI machines in clinical use. There is also no contraindication to having a CT scan after implantation of an AMS-700 penile prosthesis.

Who is a Good Candidate for AMS-700 Penile Implant?

The AMS-700 penile implant is a very versatile three-piece inflatable penile prosthesis that can be used for all patients with erectile dysfunction who desire penile implant. This type of penile prosthesis can be used in patients with erectile dysfunction caused by hypertension, diabetes, high cholesterol, obesity, renal insufficiency, and renal failure on dialysis, as well as other causes. The AMS-700 penile prosthesis is also a good option for patients with erectile dysfunction associated with Peyronie’s disease and penile fibrosis and scarring.

Patients with erectile dysfunction caused by radical prostatectomy or radiation therapy for prostate cancer as well as treatment for bladder cancer with radical cystectomy or radiation are also good candidates for the AMS-700 penile implant as the reservoir can be placed in an ectopic position.

What Other Options are Available for Patients Desiring a Penile Implant?

Patients desiring a penile implant have 3 options available: They can have a three-piece inflatable penile implant, which is often the preferred option for many patients.

For patients who have had prior pelvic surgery and have extensive adhesions, a two-piece inflatable penile prosthesis, the AMS Ambicor penile implant (an inflatable penile prosthesis without a reservoir) may be a good and safe option.

A  concealable malleable penile implant such as Tactra penile prosthesis, Spectra penile implant, and Genesis malleable implant is another option for patients who do not have good manual dexterity and want simplicity. The Spectra penile prosthesis is a good option for many patients with arthritis and limited dexterity.
