Testis that Pull Up in the Groin: What You Need to Know<\/strong><\/h1>\n

A retractile testicle<\/strong> is a testicle that tends to ‘retract’ or migrate up high in the scrotum or the groin<\/strong>, sometimes completely but temporarily disappearing from the scrotum for a period of time. This may occur with activity, during periods of emotional stress, or randomly.  Sometimes, this is accompanied by a ‘shrinking scrotum<\/strong>‘ as well.  Positional discomfort<\/strong>, in certain positions, can occur when the testicle pulls up into the groin.  Technically, the testicle can travel as high as the external inguinal ring, sitting between the thigh and the penis. Occasionally, pain may also radiate to the penis.<\/p>\n

What is the Cause of “Retractile” Testis?<\/h2>\n

A testicle is pulled up or ‘retracts’ high in the scrotum due to the strong pull by the surrounding muscles, called cremasteric muscles or cremasteric muscle fibers. Normally, the testicle pulls up in cold weather, along with the scrotum. It also ‘pulls up’ during ‘fight and flight responses, an evolutionary reflex to preserve the testis from injury.<\/p>\n

In some men, this reflex is “hyper-responsive or hyperactive<\/strong>” The cremaster muscles are activated in situations when they should not and with greater force than they should, causing pain or discomfort.<\/p>\n

What is the Treatment of Retractile Testis?<\/h2>\n

A retractile testis may not need any treatment<\/strong> if they do not cause pain or discomfort. Many children outgrow retractile testis and they get pulled less actively as children become older and as adults get older.<\/p>\n

However, some men experience significant discomfort or pain<\/strong> when the testicle pulls up high in the scrotum and into the right or left groin. This may occur at rest, in certain positions, in colder weather, during sex, playing sports, or in stressful situations.<\/p>\n

When a hyperactive cremasteric muscle response leads to the retractile testis being pulled up in the scrotum or the groin and causes discomfort, a few treatment options are available.

Surgical treatment of retractile testis involves lysis of cremaster muscle fibers<\/strong><\/a> surrounding the spermatic cord.  This can be performed with magnification as a microscopic cremasteric muscle release.  The incision is made at the level where the testis is typically pulled sublingually.  The recovery is fast; most men can resume regular activities within a few days but may experience pain for 1-2 weeks while the area is healing.  This can be performed in our office under local or general anesthesia.
