What is a Varicocele?  <\/strong><\/h1>\n

\"\"Varicocele<\/strong> <\/a>refers to enlarged veins in the scrotum<\/strong>. They are similar to spider veins<\/strong>\u2019 on the legs. Varicoceles are caused by abnormal pooling of blood in the veins of the scrotum.<\/p>\n

Varicocele can often be felt on palpation and is often described as a \u201cbag of warms<\/strong>.\u201d  Varicoceles are sometimes only felt with Valsalva maneuver or exertion, such as lifting weights, walking, or coughing.<\/p>\n

Some varicoceles grow big enough to be seen with the naked eye through the skin of the scrotum, especially in an upright (standing position).  Varicoceles that can be felt or seen are called  \u201cClinical\u201d varicoceles<\/strong>.<\/p>\n

Some varicoceles are discovered on ultrasound of the scrotum and testes but cannot be seen or palpated.  These are called \u201csubclinical varicoceles<\/strong>.\u201d  Subclinical varicoceles are usually not treated a<\/u>s they rarely cause symptoms.  However, in obese men or men with very thickened scrotal skin, where palpation may be difficult, varicoceles discovered on ultrasound may actually be \u201cclinical\u201c varicoceles and may benefit from treatment.  Some men with subclinical varicoceles and abnormal semen parameters or infertility may benefit from varicocelectomy.<\/p>\n

What are the Treatment Options for Scrotal Varicoceles?<\/h2>\n

Microscopic varicocelectomy<\/a>, laparoscopic varicocelectomy as well as embolization are some of the common treatment options for varicoceles.<\/p>\n

Varicocelectomy for Treatment of Chronic Testicular and Scrotal Pain.<\/strong><\/h2>\n

Scrotal varicocele is one of the more common causes of chronic orchialgia or testicular pain and discomfort<\/a>.  While there are many causes for scrotal and testicular pain, many causes are temporary, related to infection, such as epididymitis<\/a>, epididymo-orchitis<\/a>, referred pain, musculoskeletal causes due to pulled ligaments or muscle.  These tend to disappear spontaneously over a period of a few weeks to a few months.<\/p>\n

In men with persistent or recurrent scrotal or testicular pain that has been present for many months, if a varicocele is present, treatment of varicocele with varicocelectomy has been shown to effectively treat testicular pain in the majority of men<\/strong>.  Among men with varicocele and scrotal pain, duration of pain greater than 3 to 6 months is the best predictor of a successful outcome.  75-80% of men with varicoceles experience complete or partial resolution of scrotal pain after varicocele repair.<\/p>\n

Do I Need to Worry About Varicoceles?<\/strong><\/h2>\n

Some varicoceles do not cause any problems or symptoms.  These may not need to be treated.  However, accumulating evidence suggests that over time varicoceles, even if otherwise asymptomatic, may lead to decreased testosterone levels.<\/p>\n

Teenagers and Adolescents<\/strong><\/h2>\n

In teenagers and adolescents<\/strong>, varicoceles may impair testicular growth<\/strong>. If there is uneven testicular growth, varicocele repair or varicocelectomy is advised.<\/p>\n

Adults<\/strong><\/h1>\n

Scrotal Discomfort, Testicular Ache, and Pain<\/strong><\/h2>\n

Varicoceles may cause groin and scrotal discomfort, testicular ache<\/strong>, and pain.  While there are many causes of scrotal pain, chronic pain and discomfort for greater than 3-6 months are more likely to respond to treatment.<\/p>\n

Low Testosterone Levels (Hypogonadism).<\/strong><\/h2>\n

Men with varicoceles tend to have lower testosterone levels<\/a><\/strong> than men without varicoceles.  Accumulating evidence suggests that the repair of varicocelectomy leads to an average increase of testosterone levels by 100 ng\/DL<\/strong>.  Overall, 70% of men who have varicocele and low testosterone levels experience an increase in testosterone levels after varicocelectomy.<\/p>\n

Infertility<\/strong><\/h2>\n

Men with infertility caused by low sperm count, low sperm concentration, low sperm motility, and abnormal forms experienced significant improvement in semen parameters after varicocelectomy<\/strong>.<\/p>\n

Treatment Option for Varicocele Repair<\/h2>\n

Several options are available for varicocele repair,<\/strong> including surgical varicocelectomy and endovascular treatment (varicocele embolization).  Surgical treatment is the most established treatment with a long history of success, but embolization can be successful for many men.<\/p>\n

In general, surgical treatment<\/strong> can be performed with an open procedure, called open varicocele repair, laparoscopic varicocele repair, and microscopic varicocelectomy.<\/p>\n

Regardless of the approach (open, laparoscopic, or microscopic), the procedure can be performed as a same-day procedure with the patient going home the same day after the procedure.  Most men can return to work within 1-2 days after the surgery.<\/p>\n
