He reports a history of ED with difficulty obtaining and maintaining erections for many years. He previously used Viagra but it stopped working<\/a><\/strong>. Over the past 3 years, he has been using penile injections with Trimix<\/a><\/strong> which worked well until the past 6 months. Over the past 6 months, he noticed a lump in the penis<\/a><\/strong> and progressive penile curvature up and to the left<\/a><\/strong>. Trimix injections are not working as well as they did previously with 70% erection hardness compared to 90% before.<\/p>\n The patient reports that he saw another urologist and was prescribed Vitamin E<\/a> <\/strong>which has not helped.<\/p>\n The patient was evaluated. Blood work revealed normal testosterone<\/strong>. He was noted to have elevated blood pressure. <\/p>\n Penile Doppler ultrasound<\/a><\/strong> revealed compensated vascular insufficiency<\/strong> and no evidence of veno-occlusive insufficiency. Injection of Trimix<\/strong> resulted in a 75% erection that lasted 20 minutes. <\/p>\n Physical exam revealed a palpable penile plaque 2.5 cm long in the mid-to-distal penis<\/strong>.<\/p>\n Penile curvature measurements with the penis fully erect<\/a><\/strong>): 40-degree dorsal curvature (up)<\/strong>. No significant hourglass (narrowing) appearance<\/strong> was noted.<\/p>\n The patient was advised of the following treatment options for Peyronies disease<\/a><\/strong> available to him: observation, penile traction therapy<\/a>, injections with Xiaflex<\/a><\/strong>, verapamil<\/a><\/strong>, or interferon<\/a><\/strong>. Relative risks and benefits of each treatment approach were discussed. <\/p>\n At the follow-up visit 1 month later<\/strong>, the patient does not want to be dependent on penile injections long-term. He wants to proceed with a penile implant with possible plaque excision and grafting to address both penile curvature and erectile dysfunction at the same time. <\/p>\n The patient underwent penile implant surgery<\/strong><\/a> 2 weeks prior to the visit. His curvature was treated with penile modeling<\/a><\/strong> and he has minimal residual curvature. He was taught how to inflate the prosthesis.<\/p>\n The patient is 6 weeks after penile implant. He is able to inflate and deflate the implant. The incision is well-healed. He was advised that he can use the implant for sexual activity<\/strong>. <\/p>\n He is satisfied with his choice of a penile implant<\/strong> as the treatment for ED and Peyronie’s disease. He is using the implant successfully for sexual activity.
