He noticed a lump in his penis<\/a><\/strong> 2 years ago. The patient reports no history of trauma. No pain with erections. The patient reports no significant change in penile curvature over the past 6 months. The patient has no significant difficulty with penetration but the curvature bothers him to a great extent. The patient was treated with Gainswave shockwave therapy<\/a><\/strong> to the penis and platelet-rich plasma injections in the penis; neither treatment provided any benefit. The patient also complains of penile narrowing<\/a><\/strong>.<\/p>\n A physical exam<\/strong> revealed a palpable plaque in the midshaft of the penis.<\/p>\n Peyronie’s Disease Questionnaire (PDQ)<\/a><\/strong> score = 11. SHIM score of 17. Erection Hardness score of 3. <\/p>\n The patient was evaluated. Blood work revealed normal testosterone<\/strong> and elevated LDL. <\/p>\n Penile Doppler ultrasound<\/a><\/strong> revealed compensated vascular insufficiency<\/strong> and no evidence of veno-occlusive insufficiency. Injection of Trimix<\/strong> resulted in >80% erection. <\/p>\n Measurements were performed with the penis fully erect<\/a><\/strong> (>80% erection): 50-degree dorsal curvature (up) 6<\/strong> cm below the corona. Moderate hourglass (waist) appearance<\/a><\/strong> was noted.<\/p>\n The patient was offered tadalafil daily to improve his erection quality.<\/p>\n The patient was advised of the following treatment options for Peyronies disease<\/a><\/strong> available to him: observation, penile traction therapy<\/a>, injections with Xiaflex<\/a><\/strong>, verapamil<\/a><\/strong>, or interferon<\/a> (interferon is no longer available in the US as of 2022)<\/strong>. Surgical treatment options, including penile plication<\/a><\/strong> and a penile implant<\/a>, <\/strong>were discussed. The relative risks and benefits of each treatment approach were discussed. <\/p>\n The patient chose to start treatment with penile traction therapy. He was concerned about the side effects of Xiaflex and chose to try Interferon injections.<\/p>\n The patient started using a penile traction device<\/strong>. He started treatment with Interferon injections with penile block. He felt that Cialis had helped him with erections but experienced headaches. Tadalafil (Cialis) was changed to sildenafil.<\/p>\n The patient had received 6 injections of interferon<\/strong> while using penile traction therapy. He feels that there is a significant improvement in the curvature. <\/strong>He reported good erections with sildenafil and no side effects.<\/p>\n Artificial erection was performed and measurements were obtained. His curvature has improved from 50 degrees to 35 degrees<\/strong>. <\/p>\n The patient had a persistent hourglass appearance (narrowing of the penis). Treatment options for hourglass appearance, which include plaque excision or penile implant, were explained to the patient.
