PSA (Prostate-Specific Antigen) Test: What You Need to Know<\/strong><\/h2>\n

The PSA test<\/strong> is a blood test that is often obtained in men with an enlarged or abnormal prostate, blood in the urine, or when men have difficulty urinating.  PSA levels correlate with the size of the prostate and biological activity within the prostate.  The larger the prostate, the higher the PSA test values tend to be.  PSA tends to increase as men age and as the prostate grows in size.<\/p>\n

You have a High PSA or an abnormal PSA Test. What are the Options?<\/strong><\/h2>\n

\"shutterstock_72351682\"For men with elevated or abnormal PSA test results, it is important to determine the risk factors and the PSA level in absolute terms.<\/p>\n

Depending on risk factors and the exact PSA test result, either active surveillance with repeat PSA<\/strong> testing<\/span> or prostate biopsy<\/strong> would usually be the options. Newer tests, such as PHI (Prostate Health Index) and 4KScore, can also be obtained to glean more information.<\/p>\n

Multiparametric MRI (mMRI)<\/strong> and ExactVu<\/strong> Micro-ultrasound can help detect areas in the prostate that are suspicious for prostate cancer. Biparametric MRI (bpMRI) has also been shown to provide useful results.<\/p>\n

A number of experimental approaches are being used, including prostate MRI and a number of other tests, but these have not been shown to be of significant benefit.  It should be noted that unless there is a urinary infection and active prostatitis, treatment with antibiotics to lower PSA values is not recommended.  Well-designed studies have shown that in men with elevated PSA, treatment with antibiotics provides no benefit.<\/strong><\/p>\n

Because PSA fluctuates day to day and may be elevated due to infection or other factors, it is recommended that an elevated PSA be confirmed a month later or a month after treatment for a UTI.<\/p>\n

A Better Way: New Options for Men with Elevated PSA<\/h2>\n

While elevated PSA might mean an increased risk for prostate cancer, many men with a slight elevation of PSA do not have prostate cancer.<\/p>\n

There is a number of useful pieces of information that can be obtained right in our office, such as Total and Free PSA<\/strong>, as well as PSA density.  PSA density<\/strong> adjusts the PSA value for the size of the prostate. A large prostate is expected to have a higher total PSA than a small prostate.<\/p>\n

A newer generation of tests, including PHI (Prostate Health Index)<\/a> and 4KScore, <\/a>provides additional valuable information to help urologists decide which men are at risk for aggressive prostate cancer. There are many other tests that provide similar information.  In general, more testing is not always helpful and can produce discrepant results.<\/p>\n

PSA as a Test for Prostate Cancer<\/h2>\n

PSA has been found to be increased in men with prostate cancer. It has been used as a test to detect prostate cancer risk<\/strong> in men with symptoms that may be caused by prostate cancer.  PSA testing has also been used as a screening test for the early detection of prostate cancer<\/strong>.<\/p>\n

What is the Controversy about PSA Screening?<\/h2>\n

There is no controversy<\/strong> that men with urinary symptoms such as frequent urination, slow urine stream, blood in urine, back pain or pelvic pain, or signs such as abnormal prostate should have PSA checked.  In this situation, PSA has been shown without question to benefit men by helping to detect prostate cancer.<\/strong><\/p>\n

The controversy lies in the early detection of prostate cancer among perfectly healthy men without any symptoms.<\/strong><\/p>\n

Often, the PSA test may be falsely elevated in these men; when prostate cancer is detected, it is often detected at such an early stage that it would not cause any symptoms for 10-15 years.  Because most men decide to treat cancers that may never cause symptoms, there is a question of what benefit is accrued to men from such early detection.<\/p>\n

Studies show that there are certainly men who benefit from early detection of prostate cancer with PSA screening. There are other men who do not benefit.  <\/span><\/p>\n

The question boils down to who should be treated and when<\/strong>? Dr. Alex Shteynshlyuger uses a number of innovative approaches, including active surveillance for prostate cancer<\/a>, to maximize benefits and to minimize risks to patients.<\/p>\n

The Goal of PSA Screening<\/h2>\n

The goal of PSA screening in the modern era is to separate men who will benefit from PSA screening from those who will not.  More importantly, with the introduction of more effective treatment for prostate cancer, it is necessary to decide which men should have what treatment when prostate cancer is detected.<\/b><\/p>\n

Improved Options for Prostate Cancer Screening<\/h2>\n

To overcome the limitations of PSA, a number of improved tests for prostate cancer<\/a> have been created, including the PHI <\/a>and 4Kscore<\/a> tests.<\/p>\n

Prostate MRI<\/strong> is one of the most useful tests that is routinely recommended to men with elevated PSA to detect areas of suspicion within the prostate. A targeted MRI-fusion prostate biopsy<\/a> can then be performed. For patients without a distinct abnormality on prostate MRI, high-resolution microultrasonography<\/strong> can be used to detect suspicious areas.<\/p>\n

In addition, molecular and genetic testing of prostate biopsy specimens allows us to detect high-risk disease with more certainty.<\/p>\n
