Initial visit fees, follow up visit fees for evaluation of urinary symptoms will apply. These may be covered partially or fully by your insurance company.

 

We see men and women who are uninsured and pay out-of-pocket as well as patients who have health insurance. For patients without insurance or those paying out-of-pocket, we offer affordable prices and payment options.

 

As for insurance coverage, we offer free insurance verification. We can determine both coverage and any fees before your visit. To do so, we require the following information:
1) Your MEMBER ID number
2) Your Full name
3)Your Date of Birth.

 

If you need help with paying for medical expenses or deductible payments, you may be eligible for 0% APR credit for medical and non-medical expenses. Click to Check Eligibility. We accept payments by cash, credit cards, debit cards, PayPal and Zellepay.com. Other payment options include: no-interest or low-interest credit cards, medical loans such as CareCredit. For patients who have Health Savings Accounts (HSA) or Flexible Savings Accounts (FSA) the cost of services can be paid with your HSA/FSA card.
