HPV Vaccination in Women with Positive HPV Test or Positive Pap Smear (CIN)

The 9-valent Gardasil vaccine protects against nine strains of HPV including seven strains that cause cancer. HPV types 16 and 18 are responsible for 80 percent of cervical, vaginal, and vulvar cancers in women. These two strains and 5 additional strains are targeted by the Gardasil-9 vaccine.

HPV vaccines can be safely given to women who have an abnormal Pap test, women with a positive HPV test, and women with visible genital warts. Studies show some benefit of Gardasil in women with established HPV-related cervical dysplasia (precancerous changes).

The HPV vaccine is recommended even if a woman has a history of genital warts or an abnormal Pap test.

Pregnant women should wait until the completion of pregnancy before getting the HPV vaccine as its effects on the fetus are not known.

Risks of Recurrent HPV Infections

Persistent and recurrent HPV infections are associated with an increased risk of cervical cancer. An additional benefit of the HPV vaccine is a potential decrease in wart occurrence and recurrence. Women in their 30s and 40s can potentially decrease the risk of transmitting the virus to new sexual partners by getting vaccinated. If a woman has a known HPV infection with visible genital warts or a positive Pap smear (PIN), it is best to have partners vaccinated and to treat visible warts

I got the HPV Vaccine. Do I Need Pap tests?

HPV vaccination with Gardasil-9 and cancer screening with Pap tests and HPV tests together reduce the incidence of cancer and promote early detection of precancerous changes in women. Studies show that vaccination with the required three doses of HPV vaccine significantly reduces the incidence of high-grade cervical abnormalities (precancerous lesions). Gardasil-9 targets the nine most common strains of the HPV virus.   

Whether or not you should get PAP tests is a complex question that depends on a variety of factors including your individual history.  Studies show that HPV testing may be more effective than traditional PAP smears for many women.  It is possible that in the future we might discover that women who received Gardasil as pre-teens before the onset of sexual activity may not need Pap smears or may need them only rarely.  It is important to continue getting Pap tests based on recommendations of your GYN for cervical cancer screening even after completing the recommended three shots of the vaccine because the vaccine does not protect against all types of the virus.
