Gardasil for Penile Cancer Prevention

Does HPV Vaccine Reduce Risk of Penis Cancer in Men Over 45?

Gardasil has the potential to decrease the risk of penile cancer and oral cancers caused by HPV, although these cancers are fairly rare in the United States. Low-risk and high-risk HPV strains can co-exist in the same person.

Even if someone has visible genital warts, HPV vaccination can protect against high-risk strains that cause penile and other cancers. Gardasil also decreases persistent and recurrent HPV infections which are associated with an increased risk of cancer.

When Gardasil is given at the recommended age of 12 to 45, men have a high level of protection against genital warts and HPV-related cancers such as penile cancer, anal cancers, and oral cancers.  It may also decrease the risk of prostate cancer as some prostate cancer appear to be HPV-related. In men over 45, even if you have visible genital warts, HPV vaccine might reduce the risk of penile and other HPV-related cancers.

Because these cancers are fairly rare compared to cervical cancer in women, there is less concern about cancer prevention as the reason for vaccination in men.  Gardasil-9 protects against nine strains of the HPV virus, seven of which are the most common cancer-causing types.

Because the Gardasil-9 HPV vaccine is very safe and offers good protection for problems that are otherwise incurable, in our view Gardasil-9 vaccination is beneficial for all men.  The major issue is the cost of vaccination and from a pure cost-benefit analysis it may not be cost-effective on population-level but for men who are at risk for HPV and are able to afford it, Gardasil-9 offers a good value regardless of age.

HPV vaccines are safe in men and women with visible genital warts or precancerous changes. Studies show Gardasil is beneficial in women with HPV-related cervical dysplasia (precancerous changes). It can be inferred that a similar biological benefit will accrue to men.

In gay men, persistent or recurrent HPV infections and anal warts are associated with a high risk of anal cancer. At New York Urology Specialists, we recommend HPV Gardasil vaccination for gay men of all ages.
