What is Ureaplasma?

shutterstock_123130195Ureaplasma urealyticum and other ureaplasma species are bacteria that are typically sexually transmitted. Ureaplasma is one of the most common infections in sexually-active men and women.  It is often under-recognized because men are rarely tested for it.

Symptoms of Ureaplasma in Men

Infections with ureaplasma often cause no symptoms in men and women. When they do cause symptoms in men, urethritis is the most common symptom. Men would typically have burning or pain when they urinate, they may also experience frequent urination, slow urine stream or difficulty emptying the bladder.

Ureaplasma Testing in Men in NYC

Ureaplasma testing is recommended in all men at risk for sexually transmitted diseases. Because many men have no symptoms, routine testing is necessary to detect ureaplasma. Urethral swab or urine sample can be used to test for ureaplasma.

There are various ways to test for ureaplasma. Urine or semen culture are reliable tests but do not always show bacteria even if the patient has an infection (false-negative test result).  We use the DNA-based identification of ureaplasma which is a very precise test with a high rate of detection of ureaplasma in men who are infected.

Typically, we test men for all other commonly tested sexually transmitted infections including chlamydia, gonorrhea, syphilis, HIV, and hepatitis when we test for ureaplasma.

Treatment of Ureaplasma in Men

Ureaplasma can be effectively treated and cured with a course of antibiotics. Antibiotics such as azithromycin and doxycycline are effective about 70% of the time.  One course of antibiotics cures about 70% of men.  It is important to confirm cure since as many as 1/3 men are not cured by the first antibiotic and require additional treatment.

It is also important that your sexual partners are treated at the same time and you abstain from sex or use condoms until both partners test negative for ureaplasma or mycoplasma, otherwise you can give it back and forth to each other.

What to Do if Antibiotics for Ureaplasma are Not Working?

Antibiotics are effective in 70% of men with ureaplasma. In 30% of men, one course of antibiotics does not cure the infection, or the infection may come back within a few days to a few weeks after treatment. 

In this situation, second-line treatment options need to be explored and the reasons why treatment may have failed need to be determined

Treatment may fail either because antibiotics were not effective, or only partially effective.  Treatment for ureaplasma may also fail if the man’s sexual partner was not treated or was not cured and got him re-infected with ureaplasma.  For this reason, testing and treatment of sexual partners are very important.
