Treatment of Incomplete Bladder Emptying Symptoms due to Prostate Enlargement in Men After Pelvic Radiation Therapy<\/strong><\/h1>\n

BPH Symptoms after Pelvic Radiation in Men<\/h2>\n

\"\"Pelvic radiation<\/strong> is most commonly used in men for the treatment of prostate cancer<\/strong><\/a> and bladder cancer<\/strong><\/a>.  These cancers typically occur in older men. Pelvic radiation is also used to treat penile cancer, testicular cancer, sarcoma, colo-rectal, and anal cancers in men.  Older men also often have enlarged prostate<\/a><\/strong> and the underlying symptoms of BPH, such as urinary frequency, slow urine stream, and incomplete bladder emptying.<\/p>\n

Radiation therapy can aggravate pre-existing symptoms of BPH<\/strong>. It may also cause new urinary symptoms<\/strong> in patients who previously did not experience them. Sometimes, prostatic swelling can be severe enough to cause urinary retention with the inability to urinate in men.<\/p>\n

Evalution of Men with Urinary Problems after Pelvic Radiation Therapy<\/strong><\/h2>\n

 For men who experience frequent urination, slow urine stream, or difficulty urinating after radiation therapy, a bladder ultrasound<\/strong> with postvoid residual<\/strong> <\/a>should be obtained to make sure that the bladder empties adequately.<\/p>\n

Urinalysis and urine culture<\/strong> should be performed to exclude urinary tract infection.<\/p>\n

Treatment of Urinary Problems in Men with Enlarged Prostate after Radiation Therapy<\/strong><\/h2>\n

Medications such as Flomax<\/a><\/strong>, Uroxatral<\/strong><\/a>, and Rapaflo are effective for the treatment of urinary symptoms attributable to an enlarged prostate in men who have undergone radiation therapy for bladder cancer, prostate cancer, or rectal cancer.  Finasteride (Proscar)<\/a> can also be used to shrink the prostate and lessen urinary symptoms.<\/p>\n

Men should be screened for symptoms of BPH before initiating radiation therapy to the pelvis. In men who develop significant symptoms of an enlarged prostate, alpha blockers<\/strong> are very effective, and most men experience significant improvement in symptoms within 1-2 weeks of starting the medications.<\/p>\n

For men who cannot tolerate medications such as Flomax and Uroxatrol or do not wish to take them, minimally invasive therapy such as Urolift procedure for BPH<\/strong><\/a> and Rezum therapy<\/strong><\/a> are available.<\/p>\n

For men who are unable to urinate after radiation therapy and require urethral catheterization, prostate enucleation<\/strong>, as well as bipolar TURP, are effective treatment options.<\/p>\n
