Dear Patient,<\/h2>\n

Our goal is to provide you with outstanding urology care. Please fill out the forms that were emailed to you when you scheduled your appointment prior to your visit<\/span><\/strong> to help us take better care of you. Contact us<\/a> with any questions you might have.<\/strong><\/p>\n

Please bring all information, including past studies if available<\/span> (ultrasounds, cat scans, MRI studies), as well as notes of surgeries, pathology results,<\/span> and physician care notes if you have seen urologists in the past. It would be preferable if you bring this information prior to your visit.<\/p>\n

The required forms will be sent to you by email<\/strong> as soon as you schedule an appointment<\/a><\/strong>.  The new forms can be filled on any device<\/strong> (phone, computer, tablet), and they are saved automatically as you work<\/strong>. You can fill them partially, close them, and come back to finish filling them in.  <\/p>\n
