Please <strong><span style=\"text-decoration: underline;\">fill out the forms that were emailed you when you scheduled your appointment prior to your visit<\/span><\/strong> to help us take better care of you.&nbsp;<strong><a title=\"Contact Us \u2013 New York Urology Specialists \u2013 Phone, Email, Fax, Text\" href=\"\/?page_id=876\">Contact us<\/a>&nbsp;with any questions you might have.<\/strong><\/p>\n<p>Please <span style=\"text-decoration: underline;\">bring all information including past studies if available<\/span> (ultrasounds, cat scans, MRI studies) as well as notes of <span style=\"text-decoration: underline;\">surgeries, pathology results<\/span> and physician care notes if you have seen urologists in the past. It would be preferable if you bring this information prior to your visit.<\/p>\n<p>The <strong>required forms will be sent to you by email<\/strong> as soon as you <strong><a href=\"\/?page_id=38\">schedule an appointment<\/a><\/strong>.&nbsp; The<strong> new forms can be filled on any device<\/strong> (phone, computer, tablet) and <strong>they are saved automatically as you work<\/strong>.
