PMSA PET Scan: What You Need to Know
PMSA (Prostate membrane specific antigen) is a cell-wall component of prostate cells. It is present on the cell wall of prostate cancer cells. Specialized radio-tracers that can “find” and attach to PMSA on the surface of prostate cancer cells allow precise detection of prostate cancer metastasis earlier than other methods such as bone scan and MRI.
PMSA PET radiotracers are labeled with a very small level of radioactive isotopes that can be detected by PET detectors.
PMSA-PET/CT or PET/MRI has increased sensitivity in detecting micrometastatic disease.
How is PMSA PET performed?
PMSA PET is performed in a radiology center specifically designed for performing PET scans. The PMSA radiotracer is produced in special factories and typically shipped overnight or on short notice to the facility. The radiotracer is injected into the patient through IV. After some period of waiting to allow the PMSA radiotracer to “find” and “attach” to prostate cancer cells, the patient is imaged in a PET detector that detects the location of the radiotracer.
The areas that have increased concentration and localization of radiotracer above the baseline level in the blood or the liver, for example, are considered to be “positive.” These areas are suspicious for metastatic spread if outside the prostate or correspond to the location of prostate cancer within the prostate.
What are the differences between PMSA agents?
There are a number of different PMSA radiotracers available.
In the US, two radiotracers are FDA approved: Illuccix (68GA-PMSA-11), or Pylarify (F-18 piflufolastat PSMA or 18F-DCFPyL PET/CT) are available options. PMSA-PET/CT or PET/MRI has increased sensitivity in detecting micrometastatic disease.
PET scans using PMSA radiotracer are usually performed in combination with a CT or MRI to allow precise correlation and localization of the increased PMSA activity to the underlying anatomy. Depending on the imaging center, their equipment may be designed to work with MRI or CT. The PET images are then fused with CT or MRI images to localize the site of increased radiotracer activity which corresponds to prostate cancer.
In general, the different radiotracers provide similar information. There are some technical differences among different radiotracers, which include their half-life, stability, and excretion.
Limitations of PMSA PET scan in Evaluation Patients with Prostate Cancer
Sometimes, a PMSA PET scan can “light” up areas of the bones that may not have metastatic disease. in 3-5% of PMSA tests, a benign bone lesion might light up on PMSA scan. In other situations, areas may light up for unclear reasons. This can occur fairly often in certain situations. An experienced urologist and radiologist would be aware of this possibility. In certain situations, additional tests may be ordered if there is a suspicion that the PMSA PET scan results are “false positive.” A repeat test with an alternative PMSA agent may be ordered to confirm the finding.
