Prostate Massage and Pelvic Floor Physical Therapy: What You Need to Know

Prostate massage is performed for diagnostic reasons to improve the detection rate of bacterial and fungal organisms from the prostate in patients with symptoms of prostatitis.  Prostate massage is usually performed for 30-60 seconds.  This pushes microbial organisms that may be present in the prostate from the prostate into the urethra.  Following a prostate massage, urine is collected for microscopic analysis and culture.  There are a few different approaches used, including the traditional Stamey four-glass test. 

Urine is collected before the prostate massage, and three different urine specimens are collected after the prostate massage.  The Meares-Stamey test is useful for research purposes. At New York Urology Specialists, we use a newer, less cumbersome approach where only 1 urine specimen is collected after the prostate massage, which contains VB2 (expressed prostate fluid) and VB3 (bladder urine).  A semen culture may also be performed to get additional information.

Prostate massage can also be performed as part of a pelvic floor physical therapy program for the treatment of pelvic pain and prostatitis.

Men with acute prostatitis often feel a relief of symptoms after the prostate massage due to the expulsion of infected fluid from the prostate.

When performed as part of pelvic floor physical therapy, prostate massage is part of the internal physical therapy treatment that also treats the muscles of the pelvic floor that surround the prostate.

In men with acute prostatitis, prostate massage can cause a fair amount of pain and discomfort. In patients with pelvic pain and chronic prostatitis syndrome, pelvic floor physical therapy and internal pelvic floor therapy that includes prostate massage tend to cause minimal discomfort.
