Testicular Torsion: What You Need to Know

Scrotal and testicular pain is a common complaint in men seen by urologists.   Testicular torsion is one of the most concerning causes of testicular pain in children and adults that can lead to loss of the testicle if not treated within 4-6 hours.  

There are many causes of testicular pain other than testicular torsion.  Whenever a child or a man experiences severe testicular pain, seeking an urgent evaluation from a urologist or an emergency room is advised.  In young children who cannot communicate pain, if the scrotum or testis is swollen, immediate evaluation at the nearest pediatric emergency room is advised.

What is Testicular Torsion?

Testicular torsion is a condition caused by an interruption of blood flow to the testes.  Most commonly, testicular torsion is caused by a twist of the testicle around the spermatic cord that contains the blood vessels that supply nutrients to the testes.  Testicular torsion can occur for a variety of reasons, including trauma and anatomical predisposition caused by Bell clapper deformity.  Most often, no obvious cause is identified.  In some families, there appears to be a predisposition to testicular torsion with multiple family members affected.

What are the Symptoms of Testicular Torsion?

Testicular torsion causes sudden and severe pain in the testes.  Occasionally, the pain is severe enough to cause nausea and vomiting. Fever is uncommon, but transient flu-like symptoms can occasionally occur during episodes of acute pain.

Other conditions, such as epididymitis and epididymo-orchitis, can cause very similar symptoms and are often difficult to distinguish from each other.

What to do if he has severe testicular pain?

Severe testicular pain may be an indication of testicular torsion, which is an emergency.  Treatment of testicular torsion usually requires surgical treatment with orchiopexy.  Men are advised to go to the emergency room immediately if they experience severe testicular pain, as time is of the essence, and delayed care by even a few hours may lead to loss of the testicle.

Who Gets Testicular Torsion?

Testicular torsion is most common in teenagers, adolescents, and men under age 30.  Younger boys, as well as adults and older men, can also experience testicular torsion.

Men who have relatives who have had testicular torsion seem to have a higher risk of testicular torsion.

What is Intermittent Testicular Torsion?

Intermittent testicular torsion refers to a condition in which the testicle twists and untwists spontaneously.  When the testicle twists, a man will experience severe testicular pain, which is often relieved spontaneously by the time the man arrives at the emergency room.  Recurrent episodes of these symptoms may indicate intermittent testicular torsion.  Because there is a significant chance of testicular torsion in the future that may not resolve spontaneously, men with symptoms suggestive of intermittent testicular torsion may be offered preemptive treatment for testicular torsion despite resolution of pain.

What is the Treatment for Testicular Torsion?

The treatment for testicular torsion involves scrotal surgery with detorsion and orchiopexy to prevent the problem from recurring.  Surgery must occur within a few hours of the onset of pain to maximize success.  Delay with surgery can lead to loss of the testicle. Presentation after 8-12 hours increases the risk of testicular loss and the need for orchiectomy (testicular removal).

A testicular ultrasound (or sonogram) is often helpful in the diagnosis of testicular pain.  Testicular ultrasound is a non-invasive, radiation-free imaging modality used to visualize the anatomy and architecture of the testis.   New York Urology Specialists offers testicular ultrasound on-premises.

What is the Source of Testicular or Scrotal Pain?

The scrotal sac contains many structures, including the testes, epididymis, vas deferens, blood vessels, and skin.  Pain in the scrotum sometimes originates in the testes but may also originate in other organs, such as the epididymis, or be caused by pain and pressure on the walls of the scrotum from an inguinal hernia, for example.  A careful physical exam can differentiate and help to establish the source of pain in order to choose the most appropriate treatment option.
