What are Scrotal Cysts?

The most common type of cysts on the scrotum is “inclusion” skin cysts. Scrotal cysts are non-cancerous, benign skin cysts that are not dangerous but can grow and occasionally can get infected. Many men find them aesthetically unappealing and wish to have them removed.  When a cyst gets infected, it often causes pain.

It is important to distinguish scrotal cysts from other lesions that occur on the scrotum that include scrotal warts, scrotal skin cancer, and varicoceles.  Scrotal cysts are different from epididymal cysts.

Removal of Scrotal Cysts at New York Urology Specialists

At New York Urology Specialists, we offer scrotal cyst removal in our New York City office.  The procedure is performed under local anesthesia with minimal to no discomfort. 

Large and numerous scrotal skin inclusion cysts may require more than one session to remove, as there is a limit on how many cysts can be removed in one sitting that depends on their size and how close they are located to each other.

Our patients are highly satisfied with the results of the removal of cysts from the penis in terms of aesthetic appearance and functional outcomes after the procedure. 

Scrotal cysts are removed in the office setting under local anesthesia.  At New York Urology Specialists, >90% of our patients choose to have scrotal skin cyst removal to be performed in the office under local anesthesia.  The procedure usually takes less than 30-60 minutes (but can take 1-2 hours for patients with idiopathic scrotal calcinosis), and the patient can return to usual activities the same day.  No sexual activity is allowed for 3-4 weeks after the procedure to allow complete healing to occur. 

Patients are able to return to work the same day or the next day.  It is necessary to abstain from sexual activity for about 2-3 weeks until the area heals completely.  Most patients do not require any pain medications after the removal of penile pearly papules.

Same-day procedure:  Removal of scrotal cysts can usually be performed the same day as your consultation.  This is a convenient option for patients who are traveling for the procedure.

What are the Causes of Scrotal Cysts?

Some men are predisposed to having inclusion cysts on the scrotum and the penis.  It is not clear why they appear, and there is no way to prevent cysts.  When penile cysts or scrotal cysts become bothersome, they can be removed. 

Common Types of Scrotal Cysts

Epidermal inclusion cysts – these are also known as follicular infundibular cysts, scrotal epidermal cysts, or epidermoid cysts.  These may have ‘cheeselike’ material inside, which can get infected; when it’s expressed, foul-smelling material may be noticed. The cheeselike substance is keratin.

Scrotal Calcinosis – the cause of scrotal calcinosis is not known, but it is suspected that it evolves from epidermal inclusion cysts.

Idiopathic Scrotal Calcinosis (SC) – a Large Number of Scrotal Cysts

Some men are affected by numerous small, medium, and large scrotal cysts that typically appear yellowish-brown when they get large enough.  While some men have dozens of scrotal cysts, others may have hundreds of scrotal cysts.  This condition is called Idiopathic scrotal calcinosis (SC).

At New York Urology Specialists, we have extensive experience treating idiopathic scrotal calcinosis.  The most effective treatment is removal, which can be performed in our office under local anesthesia.  Because cysts can recur in the same or different spots on the scrotum, our preference, whenever possible, is to perform skin-sparing scrotal cyst enucleation.  With skin-sparing scrotal cyst enucleation, we remove cysts through small openings in the skin and try to preserve as much skin as possible while removing scrotal cysts.  https://www.nejm.org/doi/full/10.1056/NEJMicm1215846

What are the Symptoms of Epidermal Inclusion Cysts on the Scrotum?

Scrotal inclusion cysts do not cause symptoms, but sometimes they can cause pain or discomfort if they get infected. Many men are bothered by them in particular because they may look unsightly to sexual partners.  

What is the Treatment of Scrotal Inclusion Skin Cysts?

For scrotal cysts that do not cause any symptoms and you are not bothered by the appearance, no treatment is necessary.  When scrotal cysts grow large and cause discomfort, irritation, or appearance problems, they can be removed. Removal of scrotal inclusion cysts is a relatively simple procedure that can be performed in our office.  It is important to understand that inclusion skin cysts may recur in the same or in different spots on the scrotal wall.

Preparation for Scrotal Cyst Removal: What to Expect

No preparation is necessary for scrotal cyst removal when performed under local anesthesia (>90% of our patients undergo scrotal cyst removal under local anesthesia). You may eat and drink normally before and after scrotal cyst removal under local anesthesia. 

Recovery after Scrotal Cyst Removal

Healing time varies but usually takes 3-4 weeks for the area to heal completely. While you may take showers the same day or the next day, we advise you not to go to the beach, pool, or take baths for 3-4 weeks until healing is complete.  Please avoid activities that involve the scrotum for 2-3 weeks, such as sexual activity, biking, etc.

You may return to work the same or the next day after scrotal cyst removal under local anesthesia.

Is it Necessary to Remove Scrotal Cysts?

Scrotal cysts pose no health risks if they are not infected.  Some men are bothered by the appearance of scrotal cysts and choose to have them removed for aesthetic reasons.  Sometimes scrotal cysts cause discomfort or pain, and removal is usually curative.
