Surgery to Correct Appearance of the Scrotum
What is Scrotoplasty?

During the scrotal reduction procedure, the scrotal skin is cut away, and excess tissue is removed. Scrotal reduction surgery is also called “scrotal lift.”
What is Scrotal Reduction Surgery?
This is a relatively minor procedure in which excess skin is removed from the scrotum. It is performed in men with excessive scrotal skin as a result of prior hydrocele, scrotal swelling and skin hypertrophy, trauma, elephantiasis, or the normal aging process.
In addition to cosmetic concerns and self-esteem issues arising from a large scrotum, enlargement of the scrotum or excess skin in the area can lead to irritation and discomfort in the upper thigh area, especially during workouts in tight clothing. The reduction procedure restores a more natural and aesthetically appealing shape and appearance to the scrotum. The scrotal reduction is also called “scrotal lift” or ‘testicular lift.”
Depending on the extent of scrotal reduction required, the procedure can be performed either in the office under local anesthesia. It can also be performed at a surgical center or hospital under general anesthesia. Recovery can take from a few weeks to a few months for the area to heal completely. Scrotal reduction surgery may be covered by health insurance in certain situations.
Scrotal Enlargement Surgery
Scrotal enlargement surgery can be performed to enlarge the scrotum. Typically we use skin expanders to slowly expand the scrotal space. Skin expanders are often used in plastic surgery to expand the skin. Alternatively, a non-surgical option for scrotal enhancement includes the use of skin fillers, such as hyaluronic acid injections, that can be used to make the scrotal skin thicker and give the scrotum a larger appearance. The injection in the scrotum may relax the scrotal muscles and make the scrotum ‘hang’ more in men who experience ‘shrinking’ scrotum when they are stressed or in cold weather.
Scrotal enlargement surgery can be performed for cosmetic reasons or to correct an anatomical abnormality.
Some men desire cosmetic enlargement of the scrotum. Some men are born with a very small, underdeveloped scrotum, also known as ‘hypoplastic’ scrotum. This commonly occurs when the testicle is absent or does not develop normally. The scrotum can also ‘atrophy’ as a result of testicular loss due to trauma or surgery.
Men who are born without a testicle or lose a testicle as a result of trauma, testicular torsion, cancer, or surgery often have an underdeveloped, small scrotum. If the scrotum is underdeveloped and a man desires a testicular implant, the implant will not sit in a low scrotal position. Scrotal expanders are placed to expand the scrotum to allow it to accommodate an appropriately-sized testicular implant.
Once skin expansion has been successful, a skin expander can be removed, and a testicular implant can be positioned in the scrotum in a low scrotal position.

Why is “Scrotal Reduction” Scrotoplasty Performed?
At New York Urology Specialists, we perform scrotoplasty scrotal reduction for patients with a large and redundant scrotum. We also perform excision of scrotal scars after prior surgery as well as the closure of scrotal wounds, including skin flaps when necessary. For patients with webbing of the scrotum, we perform penoscrotal web repair. Most procedures can be performed in the office under local anesthesia. For patients who require extensive surgery, general anesthesia in a surgical center or hospital setting may be needed.
Closure of Scrotal Wound and Excision of Scrotal Scar
For patients who have a large wound on the scrotum, most commonly as a result of a poorly healing scrotal ulcer or after excision of scrotal skin during debridement of Fournier’s gangrene, we perform reconstruction with scrotoplasty and wound closure once the area is free of infection. The experienced surgeons at New York Urology Specialists use scrotoplasty reconstruction techniques that give the best possible cosmetic results with minimal functional impairment and preservation of testicular function.
Penoscrotal Web Repair
Some men are born with a long piece of skin attaching the scrotum to the penis. This results in tethering called penoscrotal webbing. Penoscrotal web repair is a type of scrotoplasty used to repair penoscrotal webbing.
In some adult men, the junction between the scrotum and penis is indistinct, with the scrotum extending onto the underside of the penis. This is called scrotal webbing. The penoscrotal web can cause symptoms such as difficulty keeping condoms on and difficulty or discomfort with penetration during sex due to the scrotum riding up on the penis. The condition may also cause cosmetic concerns due to the turkey neck appearance of the genitalia from penoscrotal webbing.
Penoscrotal web repair can often be performed in our office under local anesthesia or at a surgical center under general anesthesia. Health insurances may cover this procedure in certain circumstances if deemed medically necessary. Typically, preauthorization is required for penoscrotal web repair to be covered by a health plan.
Recovery after penoscrotal web repair is usually a matter of weeks. Most men can return to work a few days after the procedure. Men may resume their usual activities that do not involve the scrotum. The scrotal incision usually takes 3-6 weeks to heal completely. Sexual activity can be resumed once the area heals completely, usually by week 6 after the procedure.
