Epididymal Cysts: What You Need to Know

What are Epididymal Cysts?

Hypogonadism treatment NYC

Epididymal cysts are collections of fluid in the epididymis of adult males. Epididymal cysts are fairly uncommon in children. Epididymal cysts that are connected to the epididymis and contain semen and sperm are called spermatoceles. In other words, spermatoceles are a variety of epididymal cysts.

Epididymal cysts are usually noncancerous and usually do not cause problems other than being palpable in the scrotum.

Often an experienced urologist can diagnose epididymal cysts through a physical exam. A scrotal ultrasound can confirm the diagnosis if necessary.

What are the Symptoms of Epididymal Cysts?

Most epididymal cysts are fairly small, less than 0.5 cm to 1 cm in size, and cause no symptoms at all. Occasionally epididymal cysts may cause discomfort or pain.  Epididymal cysts can become infected and cause pain and discomfort. Inflammation or fluid tension on the wall of epididymal cysts can also cause pain.

Epididymal pain and pain in the epididymal cysts may be caused by infections, noninfectious inflammation, trauma, and muscular pain. Infections that cause pain in the epididymis and epididymal cysts may be bacterial, viral, or, less often, fungal.

What are the Treatment Options for Epididymal Cysts that Cause Symptoms such as Pain or Infections?

In sexually active men, testing for sexually transmitted diseases is necessary. Empiric treatment will usually cure the underlying infection. If the cause of symptoms is non-infectious and inflammatory, anti-inflammatory medications such as Motrin may be helpful. If other options do not help, draining the fluid from epididymal cysts can also help, but the problem can recur. For patients with chronic epididymal cyst pain, removing the epididymal cyst or the entire epididymis is an option.

Epididymal Cyst Percutaneous Drainage

The procedure to remove fluid from the epididymal cyst is performed under local anesthesia, with or without ultrasound guidance. A local anesthetic is administered to numb the scrotum, epididymis, and scrotal skin.  A very small needle is used to drain the fluid from an epididymal cyst. Most patients report minimal or no pain during the procedure and require no pain medications after the procedure.  Typically, the procedure takes less than 30 minutes. Men may return to work and their usual activities the next day.

When is Surgery Performed to Remove Epididymal Cysts?

Usually, symptoms caused by epididymal cysts can be resolved without surgery. When the symptoms are persistent and severe, and other treatment options are ineffective, surgical removal of an epididymal cyst or the epididymis may be considered.

A major consideration in deciding to perform epididymectomy or excision of the epididymal cyst is a concern about infertility. There is a possibility that the patient will become infertile after surgery on the epididymis or epididymal cyst, especially if it is performed on both sides. Another option that may be helpful for the treatment of chronic testicular pain is microsurgical denervation of the spermatic cord.
