Who Treats Orchitis and Epididymo-Orchitis?

Orchitis is inflammation of the testicle. Most often, testicular inflammation is caused by an infection. The infection is typically a type of UTI (urinary tract infection) or a sexually transmitted infection type.

Orchitis can also be caused by inflammation caused by a viral infection, for example, injury to the testicle or an immune reaction.

Epididymo-orchitis is inflammation of both the testicle and the epididymis. The causes of epididymo-orchitis are similar to the causes of orchitis. Epididymo-orchitis is most commonly caused by bacterial and viral infections, immune responses, or injury.

Testicular pain and swelling are the most common symptoms of orchitis and epididymo-orchitis in men.  Severe infection can cause fevers as high as 102-103F.

Treatment of Orchitis and Epididymo-orchitis at New York Urology Specialists

It is important to perform diagnostic tests before initiating treatment for orchitis or epididymo-orchitis. Based on patient history, urine tests, including urinalysis and urine culture, are typically obtained. If the patient has a fever, blood tests, including white blood cell count and blood culture, may be obtained.

Empiric treatment is usually initiated, including injection of ceftriaxone and a course of azithromycin or doxycycline if there is a suspicion of a sexually transmitted infection. Alternatively, an antibiotic such as Bactrim may be prescribed for UTI. 

If travel history or patient’s personal history suggests atypical infection, fungal infection, and tuberculosis are known to cause orchitis.

It is important to recognize that there are other causes of testicular pain, including testicular trauma, testicular cancer, and testicular torsion. Torsion of the appendix testis and appendix epididymis may also cause symptoms of orchitis or epididymo-orchitis.

Scrotal ultrasound is helpful with a definitive diagnosis and to exclude other causes of testicular pain.

Causes of Orchitis and Epididymo-Orchitis in Men

In younger sexually active men, epididymitis and orchitis are often caused by sexually transmitted diseases such as gonorrhea, chlamydia, or nongonococcal infection such as Ureaplasma.

Orchitis and epididymitis may also be caused by incomplete bladder emptying, caused by an enlarged prostate, urethral strictures, or another cause of urinary retention.

Men who underwent prostate surgery in the past or use urinary catheters are also at increased risk of developing bacterial orchitis.

Less common causes of orchitis and epididymoorchitis include tuberculosis and fungal infections.
