Treatment of Testicular Pain in Men: What You Need to Know

Scrotal and testicular pain is a common complaint in men seen by our urologists.  Pain can be mild and occasional, or it may be severe and disabling.  For some men, the pain is severe enough that having sex, walking, or sitting is uncomfortable.

Testicular pain has many causes, including infection (orchitis), epididymal cyst pain, trauma, varicocele, testicular torsion, and hydrocele.  Pain in the testis can also occur after vasectomy (post-vasectomy orchialgia).  Retractile testis occurs when the testicle is pulled up in the scrotum, causing pain or discomfort.

Why Spermatic Cord Block is a Useful Diagnostic Test for Men with Testicular Pain.

Spermatic cord block is a diagnostic test that is performed in the office to help determine the likelihood that a surgical treatment will lead to a successful outcome.  Spermatic cord block is performed by injecting a pain medication (bupivacaine or lidocaine) in the area of the groin around the spermatic cord. This is an expertise-driven, technically challenging procedure, with the success of the procedure determined by the proper technique of the injection.   We recommend repeating the test if the initial test did not lead to a significant improvement in symptoms

A decrease in testicular/scrotal pain by >50% suggests a possibly greater likelihood of a successful outcome after spermatic cord denervation surgery.

In addition, a series of spermatic cord blocks can lead to resolution of pain in up to 20% of men and to long-term decrease in severity of pain in as many as 70% of men.

  • Trauma (Can be obvious or subtle, such as from pressure)
  • Testicular torsion
  • Spermatocele
  • Orchitis & epididymitis is inflammation of the testicles due to infection or any other condition. STDs such as gonorrhea and chlamydia are common causes of orchitis in men.
  • Hydrocele, which is the accumulation of fluid inside the scrotal sac
  • Varicoceles are tortuous enlarged veins of the testicles.
  • Hematoma, which is an accumulation of blood in the scrotal sac, usually a result of trauma.
  • Inguinal hernia, which is the protrusion of abdominal contents, mainly the intestines, into the scrotal sac
  • Referred pain from kidney stones, pelvic pain
  • Post-vasectomy pain syndrome
  • Retractile Testis caused by a hyperactive cremaster muscle that pulls the testis up into the groin, causing discomfort and pain
  • Pulled muscles or impinged nerves
  • Gangrene is basically the death of testicular tissue as a result of untreated torsion or trauma.

Testicular Pain in Men and Children: What You Need to Know

There are many causes of testicular pain. Symptoms vary from occasional mild discomfort to acute, non-remitting severe pain in the testis and the scrotum (ball-sac).  Some causes of testicular and scrotal pain are fairly innocuous, while others require urgent treatment.

The most worrisome cause of testicular pain is testicular torsion.  Testicular torsion is a urological emergency that requires prompt emergency treatment.

Treatment Options for Testicular Pain

It is important to understand the source of pain. Some men have pain in the testes; other men may have pain in the epididymis or a tender epididymal cyst. Still, other men have groin pain, inguinal pain, or referred pain from the pelvis.

A comprehensive evaluation by an experienced urologist helps to narrow down the source of testicular and scrotal pain.

Once the source of pain is determined, the cause of the pain can be determined through additional testing.

The choice of treatment is determined by the location and source or cause of testicular pain. For example, if the pain is caused by an infection, treatment of that infection will eliminate the pain. If the pain is caused by inflammation, treatment of the inflammation will help with the pain.

Some men develop chronic testicular pain that takes a life of its own. Sometimes the pain is caused by what we call “neuropathic pain.”  A specialized treatment approach is used to treat neuropathic pain.

How is the Cause of Testicular Pain Determined?

An experienced urologist will obtain a detailed medical history.  The likely cause of testicular pain is typically determined after evaluation of medical history, testicular exam, and results of the testicular ultrasound.

Physical Exam is Important in Determining the Anatomical Site of Pain

A careful physical exam by an experienced urologist can distinguish between scrotal pain, testicular pain, epididymal pain, and pain originating in other sites, such as referred pain or spermatic cord pain.  During the physical examination, urologists may also determine if varicoceles, inguinal hernia, or hydroceles are present, which can sometimes cause scrotal and testicular pain and discomfort.

Testicular Ultrasound

A testicular ultrasound (or sonogram) is often helpful in the diagnosis of testicular pain.  Testicular ultrasound is a non-invasive, radiation-free imaging modality used to visualize the anatomy and architecture of the testis.   New York Urology Specialists offers testicular ultrasound on-premises.

The causes of testicular pain vary, with some causes more common in children and others in adults.

What is the Source of Testicular or Scrotal Pain?

The scrotal sac contains many structures, including the testes, epididymis, vas deferens, blood vessels, and skin.  Pain in the scrotum sometimes originates in the testes but may also originate in other organs, such as the epididymis, or be caused by pain and pressure on the walls of the scrotum from an inguinal hernia, for example.  A careful physical exam can differentiate and help to establish the source of pain in order to choose the most appropriate treatment option.
