Why Men Undergo Testicular Removal Surgery

There are a number of reasons why a man may undergo surgery to remove one or both testis.  Surgery to remove a testicle is called “orchiectomy.”  The testicle may be removed completely, or partial removal of diseased testicle may be performed (partial orchiectomy).

In addition to orchiectomy, other related surgeries include partial orchiectomy (removal of part of the testicle), epididymectomy (removal of the epididymis), testicular cyst removal, scrotal cyst removal, scrotal reduction surgery (scrotoplasty), hydrocelectomy (hydrocele or ‘water-sac’ removal), and varicocelectomy (varicocele repair).

  1. Testicular torsion – testicle twists and loses blood supply. When the treatment is delayed, the testicle often does not survive, and men undergo orchiectomy.  Often, a testicular implant can be placed at the same time that the testicle is removed; at New York Urology Specialists, we also perform testicular implants for men who desire to have one.
  2. Testicular Cancertesticular cancer is one of the most common cancers in young men.  The finding of a suspicious testicular mass usually requires surgical removal of the testicle (“radical” orchiectomy).  For men with small masses who desire to preserve the testicle, partial orchiectomy may be an option.
  3. Testicular trauma – Most injuries to the testis are mild and heal themselves or heal after surgical repair. Sometimes the injury is too severe, requiring removal of the testicle, typically because it cannot be repaired due to the loss of blood supply.
  4. Prostate cancer – Men with metastatic prostate cancer require that their testosterone levels are non-detectable.  One way to do it is to take medications to stop testosterone production. Another way to do it is to perform a “simple” orchiectomy.  Testes produce>95% of testosterone in adult men (adrenal glands produce a small amount as well).
  5. Transgender transition – People who desire to transition to non-male anatomy due to gender dysphoria may choose to undergo transgender orchiectomy.  Transgender orchiectomy requires that patients navigate a particular set of requirements set in the WPATH Standards of Care.   This includes psychiatric evaluation and counseling, use of hormonal therapy prior to orchiectomy, and 12-month experience living in an identity-congruent gender role.
