MRI Fusion Prostate Biopsy: What You Need to Know

MRI is used to evaluate the prostate for abnormal lesions that may be at higher risk for having prostate cancer. While MRI may also be used to guide prostate biopsy, the procedure is a bit more complicated. Ultrasound is widely available. Prostate ultrasound allows real-time visualization of the prostate and guidance of the biopsy needle to the area where the biopsy needs to be taken.
MRI-Ultrasound fusion with ultrasound prostate biopsy guidance is an innovative procedure that involves merging or superimposing the data from the MRI to the real-time images of the prostate obtained from transrectal ultrasound. The first step involves mapping the MRI images to the ultrasound-acquired images of the prostate. Next, the suspicious areas on the MRI, now superimposed on the ultrasound image, are used in real-time to guide the prostate biopsy needle in real-time to the suspicious areas that need to be biopsied.
MRI-ultrasound fusion biopsy of the prostate improves the detection of high-risk prostate cancer or what is called “clinically significant” prostate cancer.
Dr. Alex Shteynshlyuger is a fellowship-trained urologic oncologist, a robotically-trained surgeon specializing in the evaluation of elevated PSA, performance of prostate biopsy, and all aspects of care for early, advanced, and metastatic prostate cancer. He is a member of the Society for Urologic Oncology and the American Society for Clinical Oncology. He is one of the pioneers of MRI-fusion prostate biopsy in New York state. He is an experienced urologist with more than 20 years of experience. He has performed hundreds of prostate biopsies, including transperineal and transrectal biopsies and MRI-fusion biopsies.
Dr. Alex Shteynshlyuger is a board-certified urologist who is experienced in treating men with abnormal PSA. He has published articles in major urology journals about PSA testing and screening. What is an MRI-Fusion Prostate Biopsy?
After prostate MRI is performed, the urologist uses special fusion software and equipment that overlays MRI and ultrasound images, allowing the urologist to biopsy the exact suspicious area. Ultrasoud is used in real time to image the prostate and through fusion of images with the MRI, allows precise targeting of the prostate lesions. This improves diagnostic precision and risk stratification for treatment planning.
When is MRI-fusion Prostate Biopsy Performed and Why?
For men whose MRI shows a PI-RADS 3 or greater lesion, MRI-fusion biopsy allows to specifically biopsy the areas of concern identified on prostate MRI.
The Prostate Imaging Reporting and Data System (PI-RADS) scores lesions from 1 to 5 based on their appearance on MRI:
- PI-RADS 1–2: Very low or low risk—clinically significant cancer is highly unlikely.
- PI-RADS 3: Intermediate/equivocal risk—clinically significant cancer may be present (risk typically around 10–20% depending on other factors).
- PI-RADS 4–5: High or very high risk—clinically significant cancer is likely (often 30–70%+).
A PI-RADS score of 3 or higher indicates an increased risk compared to lower-scoring lesions. While PI-RADS 3 lesions carry a relatively lower chance of aggressive cancer than 4 or 5, we recommend either surveillance or biopsy depending on other factors, because a meaningful subset (around 12% in some series) harbor clinically significant disease. Biopsying these lesions helps avoid missing cancers that could progress, while the targeted nature of MRI-fusion reduces the risk of over-sampling insignificant areas. Factors like age, PSA density, family history, and lesion size further refine individual risk and help us to decide whether to proceed with a prostate biopsy.
