Treatment of Bladder Discomfort and Pain in Women in New York City
The urinary bladder is located in the lower abdomen adjacent to the uterus in women. Urinary bladder stores urine. Bladder discomfort in women is a common complaint treated by urologists at New York Urology Specialists.
Causes of Bladder Pain
There are many causes of discomfort and pain in the urinary bladder in women. The most common causes of bladder pain are urinary tract infections (UTIs) and sexually transmitted diseases (STDs) which include Mycoplasma and Ureaplasma. Overactive bladder and incomplete bladder emptying may also cause bladder discomfort. Large fibroids are another cause of pelvic pain, which is often described as bladder discomfort. Kidney stones that have traveled down to the lower ureter can cause pain in the bladder and pelvic pain. Interstitial cystitis (chronic inflammation of the bladder) is a known cause of bladder pain. Rarely, bladder cancer can cause symptoms of pain in the urinary bladder.
Diagnostic Tests for Bladder Pain and Discomfort
Most causes of bladder pain and discomfort can be diagnosed in our office. Urine tests, including urinalysis and urine culture, can help identify infectious organisms causing urinary tract infections and sexually transmitted diseases.
Pelvic ultrasound, including bladder prevoid and postvoid ultrasound, can help determine the health of the bladder, check if the bladder empties completely, and visualize stones or masses in the bladder as well as an enlarged uterus.
Cystoscopy and urodynamic studies may be necessary for some women to establish a cause of bladder discomfort and pelvic pain.
Treatment of Bladder Discomfort and Pelvic Pain
For most women, the diagnosis and treatment is fairly straightforward and can be established within the first few visits. For patients who have UTIs or sexually transmitted diseases, appropriate antibiotics are prescribed. It is important to remember that antibiotics such as ciprofloxacin (Cipro) and levofloxacin (Levaquin) have serious side effects and are not typically prescribed as a first-line treatment of UTIs and STDs in women.
For patients with overactive bladder, anticholinergic medications such as Vesicare and oxybutynin can provide rapid relief. However, in patients with persistent symptoms, the use of these medications long-term is not advisable as they are associated with worsening executive functions and memory problems in older women. Typically, alternative treatment options, which include Myrbetriq, tibial neuromodulation, and Botox injections in the bladder, are effective in treating women with overactive bladder.
In women with interstitial cystitis causing bladder discomfort, an FDA-approved oral medication called Elmiron may be prescribed, but this drug takes several months to become effective. Other treatments for interstitial cystitis are available to relieve symptoms of pain and discomfort in the bladder area. It is important to remember that interstitial cystitis is a fairly rare condition. It is more likely that your pain is not a result of interstitial cystitis.
Procedures such as bladder instillation and bladder distention or techniques like TENS (transcutaneous electrical nerve stimulation) may relieve bladder pain in some women. Medications and physical therapy may also be useful for some women with pelvic pain. Surgery for bladder pain is usually the last resort if other treatments do not work.
Each patient is individually evaluated and a diagnosis is made. A custom treatment plan is made that takes into consideration a patient’s preferences and lifestyle considerations. We use scientifically-proven methods to treat urinary problems in men and women.
