What is Penile Cancer and Who is at Risk?
Symptoms of Penile Cancer
Penile cancer often manifests itself by the inability to retract the foreskin (new-onset phimosis), penile lump, hard nodule on the penis, red spots, or ulceration that does not heal. While these symptoms are not specific to penile cancer and many other penile problems can cause similar symptoms, an experienced urologist can usually distinguish cancer from other causes through examination and penile biopsy when needed.
Who is at Risk for Penile Cancer?
Penile cancer is rare among men who were circumcised shortly after birth. Penile cancer is one of the less common cancers that affects men. Men who are not circumcised are at the highest risk. Men who have been circumcised as teenagers or adults are also at risk.
How Can Penile Cancer Be Diagnosed?
Penile cancer is diagnosed by doing a biopsy of the penis in men who have suspicious lesions. Experienced urologists can typically recognize lumps/masses and lesions on the penis that look suspicious for penile cancer.
Squamous cell carcinoma is the most common penile cancer. Other types of penile cancers can occur, including sarcoma (rhabdomyosarcoma, leiomyosarcoma), melanoma, Kaposi’s sarcoma, and other cancer types.
Penile Cancer Treatment.

When penile cancer is detected early, the success of the treatment is very high.
Early penile cancer can be treated with excision. More advanced penile cancer usually requires a partial penectomy. Depending on the aggressiveness and staging of penile cancer and the presence of inguinal lymph nodes, inguinal lymph node dissection may be beneficial for many men with intermediate-grade and high-grade penile cancer.
Radiation therapy (and chemotherapy) can be used to treat penile cancer in early-stage disease, but does not provide a significant advantage over surgical treatment and may not offer equivalent benefit in the absence of surgical excision. For radiation to be sufficiently effective and comparable to surgical excision, the amount of radiation that needs to be delivered will cause erectile dysfunction. It may also cause severe local complications. Chemotherapy is usually used for metastatic penile cancer. Currently, available chemotherapy is not sufficiently effective to be used for men with penile cancer that is potentially curable with surgical excision.
