Peyronies Treatment: 58-year-old Man Treated at New York Urology Specialists

RF is a 58-year-old man with penile shortening and narrowing after prostate cancer treatment.  He was diagnosed with prostate cancer and underwent robotic radical prostatectomy two years ago.  He reports that after the surgery his erections have recovered to some extent and he is using tadalafil daily and sildenafil before sex.  His erections are not as good as prior to surgery but he is able to achieve erections that are sufficient for penetration and having sex most o the time. He complains of significant shortening of the penis and narrowing of the penis shaft. He reports no prior treatment.

Physical exam revealed fibrotic scar tissue dorsally from midshaft extending to the base of the penis. 

Blood work revealed borderline low-normal testosterone, normal cholesterol levels. His blood pressure was slightly elevated.   

Follow-up Appointment – 1 month

Penile Doppler ultrasound revealed vascular insufficiency and evidence of veno-occlusive insufficiency. The patient reported that his erections are less firm at home compared with high-dose Trimix (70% hardness).

Penile measurements were performed with the penis maximally erect (70% erection): 30-degree dorsal curvature (pointing up) toward the midshaft of the penisHourglass appearance was noted circumferentially.

The patient was advised of the following treatment options for Peyronies disease available to him:  observation, penile traction therapy, injections with Xiaflex (off-label), verapamil, or interferon.  Surgical treatment options including penile plication and penile implant were discussed.  Relative risks and benefits of each treatment approach were discussed.

The patient was advised to initiate penile traction as it can help restore some of the penile length, and may improve penile curvature. Given that only a 70% erection could be achieved in the office, there is a fairly high chance that the degree of curvature measured to be 30 degrees underestimates the true curvature if a full erection (>90%) were achieved. 

Given the presence of both Peyronie’s disease and severe ED, he was advised that penile implant with possible plaque excision and grafting may be the optimal choice to address both problems long-term.  Often, penile modeling intra-operatively and insertion of a penile implant is sufficient to address penile curvature and hourglass appearance; other times a patient may require plaque excision and graft insertion.

Follow-up Appointment – 3 months

The patient has been using a penile traction device with some improvement in erect penile length.  He wanted to proceed with scheduling a penile implant surgery.   He was advised to get a medical evaluation from his primary care doctor and a cardiologist. Expectations and recovery after penile implant were discussed.  He was advised to continue using penile traction until surgery.

Follow-up Appointment – 6 months

The patient underwent a penile implant 1 month ago.  His curvature with the implant fully inflated was greater than 60-degrees with severe hourglass appearance which did not resolve with penile modeling.  He underwent penile plaque excision and grafting at the time of penile implant.  He has not engaged in sexual activity.  He has been doing penile implant ‘cycling’, inflating and deflating the implant over the past week.  Because he underwent graft insertion, he was advised to wait longer before resuming sexual activity. He was advised that he can resume sexual activity in 1 month (2 months after the procedure).

Follow-up Appointment – 9 months

The patient is 4 months after penile implant surgery. He reports that he is very satisfied with the results. He has been engaging in sexual activity. His erection quality is better than it was prior to his prostate cancer surgery.  His curvature and narrowing have been addressed with less than 15 to 20-degree residual curvature.  His wife reports that fixing the problem has brought them closer.
