Treatment Approach to Peyronie’s disease

Peyronie's treatmentPeyronie’s disease is treated by a small number of specialists.  It is important to know that many treatments that have been tried and are still in use by some doctors are of no benefit.  These include vitamin E, colchicine, and a number of other approaches.

Early use of effective treatment approach can limit the severity of the disease and lessen the severity of the condition.

Principles of Treatment of Peyronie’s Disease

Treatment of Peyronie’s Disease is Rather Complicated. 

Treatment options depend on the following factors:  

  1. How long have the symptoms been present?  Usually, surgery is reserved for patients who have had Peyronie’s for at least 1 year and no change in symptoms for at least 6 months.  There are some exceptions.
  2. If the patient’s symptoms are interfering with function (ability to have sex) by making penetration difficult or impossible, by causing pain during sex.
  3. Quality of Erections – good erections allows for more treatment options. Erections that are not adequate with use of Viagra or Cialis, or injections of Trimix/Prostaglandin E1, usually would require penile implant surgery.
  4. Presence of penile narrowing ‘waist’ or ‘hourglass” appearance – if the narrowing is severe, surgical treatment with graft insertion is usually necessary to address the problem.
  5. Penile plaque: whether it can be felt (palpable or not?), Peyronie’s plaque size, and whether it’s calcified (hard) 

Limit Progression of Early Disease

  1. Agents such as  CoQ10 have the potential of limiting the severity of the disease.  They are used in early-stage disease given a good safety profile.  They do not treat the disease.
  2. Early initiation of adjunctive treatments can help to reverse and prevent Peyronie’s progression.  These include traction therapy, injectable therapy with verapamil, collagenase, and interferon.
  3. Address pain and erectile dysfunction.

What You Should Know – Controversies in Peyronie’s Disease Treatment

Many specialists that treat Peyronie’s disease advise no active treatment for men with new-onset Peyronie’s disease or to men with minimal symptoms. The logic is that a small number of men (10-20%) will see spontaneous resolution or improvement in symptoms of Peyronie’s disease without treatment.  Some men do not experience improvement but don’t experience significant worsening either.   The problem is that some men experience severe progression of the symptoms with significant penile shortening, severe curvature, and narrowing.  Treatment of severe Peyronie’s disease is challenging. 

An Ounce of Prevention is Worth a Pound of Cure

Since close to 80% will see no improvement, I advocate proactive steps early in the disease process to decrease the chances of disease progression. While there are no studies that point to the success of this approach, we do know that every therapy we use improves the symptoms of Peyronie’s disease.  It is well established in general that it is easier to prevent diseases than to treat them. In the case of Peyronie’s we know that the treatments are effective for severe disease – it is reasonable to conclude that they are very likely to be beneficial earlier in the disease process.

Use Proven, Effective Treatment Options for Stable Peyronie’s Disease

A combination of traction therapy and plaque injections with Xiaflex, verapamil, or interferon is the initial approach for most patients with good erectile function and a palpable penile plaque.  If the benefit is insufficient, surgical treatment with penile plication, Peyronie’s plaque excision, or penile implant is initiated.  Many men who come to us for a second opinion have been treated by ineffective therapies.

Treatment Decisions in Peyronie’s Disease

Treatment options for men with Peyronie’s disease are fairly complicated and depend on a variety of factors such as duration of Peyronie’s disease symptoms, the presence of a palpable plaque; size and hardness of Peyronie’s plaque, the presence of “hourglass deformity” or narrowing of the penis, and quality of erections.
