Peyronies Treatment: 51-year-old Man Treated at New York Urology Specialists

JG is a 51-year-old man. The patient presents for the second opinion for penile curvature.  He reports a history of penile trauma during sex and sensation of something snapping in the penis more than one year ago.  The patient reports that he started noticing curvature in his penis about 18 months ago; he also has noticed a shortening of the penis but no functional difficulty with penetration.  The patient reports no prior treatment.  Patient reports worsening of the curvature in the past 6 months.

He reports difficulty with obtaining and maintaining erections for the past 3 years.  The patient also reports decreased erection hardness.  The patient has been using Cialis for the past 2 years with good benefits and no side effects.  Shim score of 21.  Erection hardness score of 3.

Physical exam revealed a palpable penile plaque in mid-penis.

Follow-up Appointment – 1 month

The patient was evaluated. Blood work revealed low testosterone, confirmed with a second early-morning testosterone level. 

Penile Doppler ultrasound revealed compensated vascular insufficiency and no evidence of veno-occlusive insufficiency.  Injection of Trimix resulted in >90% erection that lasted 40 minutes. 

Measurements were performed with the penis fully erect (>90% erection): 75-degree dorsal curvature (up) and 15-degree rotation to the left with curvature at midshaft, 5cm below the corona.  Moderate hourglass (waist) appearance was noted to be present.

The patient was advised of the following treatment options for Peyronies disease available to him:  observation, penile traction therapy, injections with Xiaflex, verapamil, or interferon.  Surgical treatment options including penile plication, plaque excision with grafting, and a penile implant were discussed.  Relative risks and benefits of each treatment approach were discussed. 

The patient chose to start Xiaflex therapy. He is a good candidate for Xiaflex given that (1) there is a palpable scar (plaque) into which Xiaflex can be injected, and (2) his curvature is greater than 30 degrees.  Insurance reauthorization was obtained based on these facts.

Follow-up Appointment – 3 months

The patient started using a penile traction device but stopped using it after 2 months. He reports no change in curvature.  He wants to start Xiaflex injections.

Two injections of Xiaflex were administered 5 days apart. The patient was advised not to engage in sexual activity for a month.  The patient was advised to use a penile traction device.

Follow-up Appointment – 5 months

The patient had pain after Xiaflex but noticed improved curvature, “almost straightened out” after Xiaflex.  He is satisfied with the results of Xiaflex and does not want to proceed with additional Xiaflex treatment. 

Follow-up Appointment – 1 year and 2 months later

The patient presents with complaints that his curvature has recurred over the past few months.  He wants to get additional Xiaflex injections as it has worked well for him.

Two additional cycles of Xiaflex were administered.  On follow-up, the patient reports noticeable improvement in curvature.  Measurements in the erect state revealed a residual curvature of 40 degrees.

He is satisfied with the results of his treatment.  He does not want to get additional treatment at this time.
