Peyronies Treatment: 53-year-old Man Treated at New York Urology Specialists
TE is a 53-year-old man with complaints of penile curvature for the past 6 months. He recalls possible injury during sex shortly before noticing the curvature. Currently, he has no pain with erections. He reports no significant change in curvature of the past 3 months. He has no functional limitations with intercourse. He is also bothered by the appearance. He reports good erections. No prior treatment for Peyronie’s disease.
He has experienced slight difficulty maintaining erections over the past 4-5 years and occasionally uses sildenafil.
Physical exam revealed a palpable plaque 2 cm long dorsally in midshaft of the penis.
Peyronie’s Disease Questionnaire (PDQ) score = 8. SHIM score of 19. Erection Hardness score of 4.
Follow-up Appointment – 1 month
The patient was evaluated. Blood work revealed normal testosterone, normal lipid profile. His blood pressure is normal while taking antihypertensive medications.
Penile Doppler ultrasound revealed normal vascular function with no evidence of vascular insufficiency and no evidence of veno-occlusive insufficiency. Injection of Trimix resulted in >90% erection.
Measurements were performed with the penis fully erect (>90% erection): 25-degree dorsal curvature (up) at the mid-to-distal shaft, 3 cm below the corona. Mild hourglass (waist) appearance was noted to be present in the same area.
The patient was advised of the following treatment options for Peyronies disease available to him: observation, penile traction therapy, injections with Xiaflex, verapamil, or interferon. Surgical treatment options including penile plication, plaque excision, and a penile implant were discussed. Relative risks and benefits of each treatment approach were discussed.
Because the curvature is of new-onset and less than 30-degrees, he does not satisfy insurance criteria for Xiaflex treatment. He was advised to start penile traction therapy. He was given the option of starting penile injections with verapamil or interferon in an effort to decrease the chance of disease progression. Given that the problem is mild and present for less than a year, surgical options are not advisable.
The patient chose to start treatment with interferon and penile traction therapy.
Follow-up Appointment – 2 month
At the follow-up visit 1 month later, TE reported that he is using penile traction therapy but has some difficulty maintaining the device in place. He had mild side effects to interferon the evening after injection but felt better the next day.
Follow-up Appointment – 4 months
The patient is using a penile traction device. He received 6 injections of interferon into the penile plaque. He has not noticed any change in his curvature. He was advised to continue using penile traction.
Follow-up Appointment – 6 months
The patient reports that he noticed a worsening of penile curvature and hourglass appearance. He also has difficulty with intercourse due to penile curvature He is using penile traction.
Follow-up Appointment – 7 months
Penile ultrasound and artificial erection were performed again. Measurements were performed with the penis fully erect (>90% erection): 55-degree dorsal curvature (up) at the mid-to-distal shaft and a 25-degree left lateral component were noted, 3 cm below the corona. Previously, the patient had a curvature of 25-degrees. Moderate hourglass (waist) appearance was noted dorsally and on the left.
Treatment options were discussed again. Given that patient has Peyronie’s disease progression but good erections, surgical treatment is not advisable at this time. The patient decided to proceed with Xiaflex treatment. He is a candidate for Xiaflex because he has a dorsal curvature of greater than 30 degrees.
Follow-up Appointment – 10 months
The patient had received 2 cycles of Xiaflex (4 injections). He experienced some swelling and black-and-blue around the penis that typically went away within 1-2 weeks after injections. He is continuing penile traction therapy.
Follow-up Appointment – 1 year
The patient had received 2 cycles of Xiaflex (4 injections), last injection about a month ago. He feels that there is an improvement in the curvature but he still experiences difficulty during intercourse and very unhappy about the appearance of the penis.
Follow-up Appointment – 1 year and 2 months
Measurements were performed with the penis fully erect (>85% erection): 45-degree dorsal curvature (up) at the mid-to-distal shaft and a 25-degree left lateral component.
After Xiaflex treatment, dorsal curvature has improved from 55-degrees to 45-degrees. His left lateral curvature has not changed significantly. The patient wanted to know about other options as he is still significantly bothered by the curvature. Surgical treatment options were discussed with the patient. Given that he has good erections with the use of sildenafil, he chose to have penile plication which offers good success rates without the risks of worsening ED that is associated with plaque excision. He was advised to wait a few months before proceeding with surgical treatment to make sure there is no further change in penile curvature.
Follow-up Appointment – 1.5 years
The patient reports no changes in penile curvature. He wants to proceed with penile plication.
Follow-up Appointment – 3 months later
The patient underwent penile plication under local anesthesia over a month ago. The incision healed well; he reports mild discomfort in the area of incision that has been getting better over the past few weeks. He reports nearly straight penis appearance. He was advised that he can resume sexual activity in 3 weeks.
Follow-up Appointment – 5 months later
3 months after penile plication. The patient has no pain, no other complaints. He is very pleased with the results. While the penis is not completely straight, there is a 90% improvement. He has no difficulty with intercourse or penetration.
