Treatment for Peyronie’s Disease after Prostate Surgery: What You Need to Know

Treatment for prostate cancer with radical prostatectomy as well as radiation is a suspected risk factor for the development of Peyronie’s disease.
Prostate removal with radical prostatectomy is associated with the development of several symptoms of Peyronie’s disease including loss of penile length, scar formation and fibrosis of the penis, Peyronie’s plaque, and penile curvature. Studies report a 15% chance of Peyronie’s disease diagnosis within a few years after radical prostatectomy.
Treatment of Peyronie’s disease after Radical Prostatectomy for Prostate cancer
It is important to understand that many men experience erection difficulties after radical prostatectomy. Typically recovery of erectile function can take 6 to 9 months.
Many men require medications such as tadalafil (Cialis) or Viagra to get an erection after treatment for prostate cancer. For some, oral medications do not work and penile injections are prescribed. Penile injections are associated with a risk of Peyronie’s disease. The exact cause and timing of the onset of Peyronie’s disease are often difficult to determine. Men tend to get diagnosed with Peyronie’s disease 1 to 3 years after their treatment of prostate cancer. The diagnosis of Peyronie’s disease is often incidental during the evaluation of erectile dysfunction.
With rare exceptions, plaque excision alone is not a viable option for men with Peyronie’s disease after treatment for prostate cancer due to the presence of underlying erectile dysfunction.
For men who are able to obtain adequate erections with the use of oral PDE-5 inhibitors such as Viagra and Cialis (tadalafil and sildenafil), and who have a palpable plaque, Xiaflex may be tried along with penile traction therapy.
For men with severe hourglass appearance, significant erectile dysfunction will nonpalpable plaque, penile implant usually offers the most effective treatment. We have successfully treated many men with penile implants.
