Penile traction therapyPenile traction therapy refers to the mechanical stretching of the penis.   To help with the task, numerous devices designed to stretch the penis and keep it stretched for prolonged periods of time have been produced.

What is Penile Traction used for?

Penile traction therapy is used for a number of reasons.

Penile traction therapy has been shown to increase penile length but 0.5-1.5 cm on average.  Because penile shortening is a significant problem for men with Peyronie’s disease, penile traction therapy can potentially prevent penile shortening or reverse it to some extent in some men.

Penile traction has also shown to help with straightening penile curvature caused by Peyronie’s disease.  It is typically used in combination with penile plaque injection therapy.

Is there a Rational Reason for Using Penile Traction Therapy?

It is well known that many biological processes occur in response to cell stretching.  Mechanoreceptors on the wall of many cells can get activated and can lead to physiological processes.  In the case of Peyronie’s disease, there is evidence that the activation of mechanoreceptors can lead to scar re-organization.

I recommend that all patients who are undergoing penile injection therapy with Xiaflex, verapamil injections, or interferon injections use penile traction therapy as an adjunctive therapy.  Penile traction therapy is also useful after penile plication and plaque excision surgery.

How Effective Is Penile Traction for Treatment of Men with Peyronie’s Disease

Multiple studies have shown a small benefit in gaining of penile length with the use of penile traction devices. It is important to note that the success of treatment depends on the duration and frequency of use. Typically use of penile traction for 4-6 hours a day for at least 6 months is required to achieve a benefit.

Studies of traction therapy also show benefits when it comes to penile curvature.

How to Use Penile Traction?

Studies show that penile traction is only beneficial with dedicated use. Men who benefit from penile traction use it 4-8 hours a day for 6 months before they see improvements.  Some devices have been shown to be effective if worn for 0.5-1 hour per day.

When to Use Penile Traction?

I advise that penile traction therapy is initiated early as soon as signs and symptoms of Peyronie’s disease are identified and diagnosed.   The goal of early penile traction therapy is three-fold:

  1. Prevent worsening of symptoms, in particular, prevent penile shortening that affects many men with Peyronie’s disease.
  2. Reverse penile shortening that has occurred
  3. Prevent and reverse penile curvature

Currently, Medicare and private insurances do not cover the cost of penile traction therapy.  Penile traction devices have to be purchased out-of-pocket.
