Peyronies Treatment: 47-year-old Man Treated at New York Urology Specialists

RE is a 47-year-old man with complaints of penile curvature causing pain with erection and intercourse, as well as a loss of penis length.  The patient reports that pain is severe enough that he sometimes loses erections because of pain; he rates pain as 7 out of 10.  He reports that he experienced penile trauma 3 years ago during sex; he had pain for about a day at that time.  A year later he noticed penile curvature.  The patient has not tried any treatment so far. 

He reports no difficulty obtaining and can maintain erections except when he experiences too much pain in the area of curvature.  He rates erections as being “excellent”.

The patient was evaluated. Blood work revealed normal testosterone, normal cholesterol levels. His blood pressure was within a normal range.   

Physical exam revealed a palpable dorsal penile plaque.

Follow-up Appointment – 2 months

Penile Doppler ultrasound revealed no vascular insufficiency and no evidence of veno-occlusive insufficiency.  A plaque was visualized.

Measurements were performed with the penis fully erect (>90% erection): 60-degree dorsal curvature (pointing up).  

The patient was advised of the following treatment options for Peyronies disease available to him:  observation, penile traction therapy, injections with Xiaflex, verapamil, or interferon.  Surgical treatment options including penile plication, plaque excision with grafting, and penile implant were discussed.  Relative risks and benefits of each treatment approach were discussed. 

The patient chose to start Xiaflex.  He was advised to start penile traction therapy for at least 6 months.

Follow-up Appointment – 6 months

The patient received 3 cycles of Xiaflex (6 injections). He noticed some improvement in curvature and wants to continue. He is continuing penile traction therapy.

Follow-up Appointment – 8 months

The patient had received 4 cycles of Xiaflex (8 injections), last injection about a month ago.  He feels that there is an improvement in the curvature but he still experiences pain with erections during sex and unhappy about the appearance of the penis.  He also complains of penile shortening which has not reversed significantly with treatment.

Measurements were performed with the penis fully erect (>90% erection): 50-degree dorsal curvature (an improvement from 60 degrees prior to treatment with Xiaflex).  

Treatment options were again discussed with the patient. He did not want to try another non-surgical therapy as the success rates are not as good as with surgery.  He decided to proceed with penile plaque excision with grafting. He is a good candidate for the procedure given the severity of his condition and good erection quality. 

Follow-up Appointment – 11 months

The patient underwent plaque excision with grafting 3 weeks ago. He was able to go home the same day. He reports having some pain for about a week in particular at night with erections when he required to take pain medications.  He has no pain now but complains of some numbness towards the tip of the penis that he notices over the past week. Swelling is still present but much improved.

He was informed that numbness is a known side effect of penile plaque excision, in particular when the plaque is dorsal. He was informed about it prior to surgery.  Usually, the problem resolves but can take 6-9 months.  

He was advised to wait another month prior to engaging in sexual activity. 

Follow-up Appointment – 1.2 years

The patient is 3 months after plaque excision with grafting. He reports that he still has some numbness but it has improved by 50-60% since surgery.  Swelling went away. He reports excellent erections.  He no longer has pain with erections and during sex that he had prior to surgery.  His curvature is dramatically improved with a slight residual curvature of fewer than 15-20 degrees.  He is satisfied with the results of surgery.   He was reassured that it is common and normal to feel permanent sutures after penile plication.

Follow-up Appointment – 2 years

The patient reports strong erections. No difficulty with penetration. No loss of penile length as a result of surgery.  The numbness went away about 6 months ago and he has a normal sensation.  He is very pleased with the outcome of treatment.
