Peyronies Treatment: 46-year-old Man Treated at New York Urology Specialists

JN is a 46-year-old man who came with complaints of penile curvature over the past 3 years. He reports no history of trauma. No difficulty with erections. He complains of difficulty with penetration and has to use certain positions to be able to engage in intercourse. His wife also complains of occasional pain during sex. There has been no change in the degree of curvature over the past year.  He was previously treated with colchicine and vitamin E without significant benefit.

The patient was evaluated. Blood work revealed normal testosterone, normal cholesterol levels. His blood pressure was within a normal range.   

Physical exam revealed a palpable ventral penile plaque.

Follow-up Appointment – 2 months

Penile Doppler ultrasound revealed no vascular insufficiency and no evidence of veno-occlusive insufficiency. 

Measurements were performed with the penis fully erect (>90% erection): 45-degree ventral curvature (pointing down) toward the distal third of the penis.  

The patient was advised of the following treatment options for Peyronies disease available to him:  observation, penile traction therapy, injections with Xiaflex (off-label), verapamil, or interferon.  Surgical treatment options including penile plication and plaque excision with grafting were discussed.  Relative risks and benefits of each treatment approach were discussed. 

The patient chose to start Interferon therapy.  He was advised to start penile traction therapy for at least 6 months.

Follow-up Appointment – 2 months

The patient started using a penile traction device and started a series of 6 interferon injections.  He had flu-like side effects and chills after the second dose of interferon.

Follow-up Appointment – 4 months

The patient completed treatment course with interferon. He reports slight improvement in curvature but he is still experiencing difficulty with penetration.  After further discussion of treatment options, he decided to proceed with penile plication.

Follow-up Appointment – 6 months

The patient underwent penile plication under local anesthesia 3 weeks ago.  He reports he had some pain with erections initially but it is getting much better.  He reports that the curvature is significantly better.  He has not engaged in sexual activity.  He was advised that he can resume sexual activity in 1 month

Follow-up Appointment – 9 months

The patient is 3 months after penile plication. He reports that overall he is satisfied with the results of penile plication. His curvature is much improved. He has not noticed any change in sensation.  He has no difficulty with penetration and sex is more satisfying.  He does report that he feels lumps in the penis, which are sutures.  He was reassured that it is common and normal to feel permanent sutures after penile plication.
