Beneficial Effect of Shockwave Therapy for the Treatment of Peyronie’s Disease-Associated Pain
Multiple studies demonstrate that shockwave therapy is an effective treatment for pain associated with Peyronie’s disease. Randomized controlled studies show a statistically significant improvement in pain compared to placebo in patients who received low-intensity shockwave therapy.
Shockwave therapy is administered in our office without the need for anesthesia. The procedure takes about 20 minutes per session. While normally shockwave therapy does not cause any pain, in patients with tender penile nodule patients typically experience some pain and discomfort that typically improves after the first few sessions.
Does Shockwave Therapy Help with Curvature associated with Peyronie’s Disease?
While there are some low-level studies without a placebo control group and that are not performed as randomized controlled studies that show some benefit of shockwave therapy in improving penile curvature associated with Peyronie’s disease, high-quality studies do NOT show any significant benefit.
It is important to remember that a small number of men will have spontaneous resolution or improvement in Peyronie’s disease symptoms. In addition, if Peyronie’s disease gets worse and affects erectile dysfunction, the penile curvature will be less noticeable and will be underestimated on subsequent measurements leading to a false result that the curvature is getting better while it actually may be getting worse.
Without high-quality randomized controlled studies with a placebo arm, it is highly likely that the studies are not providing reliable information. There is certainly a significant financial incentive for many providers including those who have no qualifications (non-urologists) when he comes to the treatment of Peyronie’s disease to promote shockwave therapy.
The bottom line: Shockwave therapy helps with penile pain. Shockwave therapy does NOT help with curvature or hourglass appearance.

It is fairly well-established based on a significant number of high quality randomized controlled studies that low-intensity shockwave therapy for the treatment of erectile dysfunction associated with vascular risk factors such as high blood pressure, high cholesterol, and diabetes leads to improvement in erection quality.
Many patients with Peyronie’s disease have risk factors for vascular erectile dysfunction including age, presence of high blood pressure, high cholesterol, coronary artery disease, or diabetes.
Peyronie’s disease also has a direct effect on erectile function in many men.
Some studies in men with Peyronie’s disease show a trend in the improvement of erectile function while other studies do not show significant benefit. This discrepancy is likely attributable to the different etiologies of erectile dysfunction in men with Peyronie’s disease.
Some men with purely Peyronie’s induced erectile dysfunction may not experience significant benefit from shockwave therapy for the treatment of severe erectile dysfunction. On the other hand, men who have combined risk factors including vascular induced erectile dysfunction and Peyronie’s induced erectile dysfunction may experience some benefit from shockwave therapy for ED.
