Low-Intensity Shockwave Therapy (Li-ESWT or LIST) is the latest innovation in the treatment of erectile dysfunction that has been shown to work in highest quality scientific studies.

Shockwave therapy for erectile dysfunction induces growth of small blood vessels (microcapillaries) which improves blood flow to the penis and results in improved erections and improved erection hardness.

Does Shockwave Therapy for Erectile Dysfunction Work for Everyone?

Shockwave therapy for ED works for many men who have inadequate erections due to poor flow of blood to the penis.  Men with vascular risk factors such as high blood pressure, diabetes, obesity, low testosterone, kidney failure on dialysis, aging, high cholesterol and history of prostate surgery are good candidates for treatment with shockwave therapy. 

Overall, shockwave therapy works for 50-70% of men with erectile dysfunction.  It also works in men with diabetes and ED.  When shockwave therapy for ED does not work, other treatment options are available.

It is important to address other causes of erectile dysfunction prior to initiation of shockwave therapy such as correcting testosterone deficiency, thyroid problems and getting medical problems such as high blood pressure and diabetes under control.  Weight loss is also helpful for men who are overweight.

At New York Urology Specialists We Define the Success of Shockwave Therapy in 3 Ways:

  1. Men who are able to get good erections without medications such as tadalafil (Cialis) or Viagra (sildenafil) after treatment with shockwave therapy, or require a smaller dose of medications.
  2. Men who are able to get better (harder) erections and can last longer after shockwave therapy in combination with medications such as Cialis daily or sildenafil (Viagra).
  3. Men for whom medications such as Viagra or Cialis, or injectable Trimix or prostaglandin did not work before treatment with shockwave but work after treatment with shockwave therapy

What do You Need to Know About Shockwave Therapy for ED?

Not all shockwave therapy is the same. There are 2 different technologies used for delivering low-intensity shockwave therapy for ED: (1) focused shockwave and (2) radial shockwave.  Focused shockwave is a more expensive way to generate and deliver shockwave energy.  Shockwave generators (devices) that use focused shockwaves such as Duolith SD1 that is capable of delivering 0.09 mJ/mm2 are the only ones that have shown to work in randomized controlled studies.  At New York Urology Specialists, we use the most effective DUOLITH SD1 shockwave generator to deliver superior results to our patients.

Radial shockwave therapy is very popular and less expensive but no randomized controlled studies to date have been performed to show that it is an effective way to treat ED.

How Long Do the Benefits of Shockwave Therapy for ED Last?

The effectiveness of shockwave therapy for treatment of erectile dysfunction depends on a number of factors including the device used, the protocol used for the treatment of erectile dysfunction, and a number of shockwaves administered as well as the frequency of treatment.  At New York Urology Specialists we use the most advanced scientifically proven protocol for treating men with erectile dysfunction using shock wave therapy.

Studies show that the beneficial effect of initial course of shockwave therapy can last for longer than 2 years in more than 50% of men who initially benefited from shockwave therapy (men received 12 treatments over 3 months).  The benefit of shockwave therapy depends on the underlying severity of erectile dysfunction and the presence of risk factors.  Patients with diabetes, severe hypertension, renal insufficiently require more frequent retreatment as do men with more severe ED.

What is Gainswave therapy for ED?

Gainswave is a trademarked term for shockwave treatment approach used by Gainswave providers.  Most providers of Gainswave appear to use radial shockwave therapy for the treatment of erectile dysfunction.  Radial shockwave therapy has a limited number of publications, and no randomized controlled studies to demonstrate effectiveness in the treatment of ED.  At New York Urology Specialists, we use focused shockwave therapy that has been shown to work in randomized controlled studies.

What are the Risks of Shockwave Therapy for Erectile Dysfunction?

Shockwave therapy for ED uses low-intensity acoustic wave energy that transmits energy to the tissues of the penis.  Some men may experience temporary mild discomfort or may develop bruising which typically disappears within 1-2 weeks.  No long-term risks have been reported from low-intensity shockwave therapy for ED as utilized in clinical protocols.  Because the delivery device is very focused, shockwaves are not delivered outside the intended area when administered by a properly trained specialist.

What You Need to Know About Shockwave therapy for ED in the US

Currently, shockwave therapy is recommended and approved for treatment of ED in some countries around the world. Strong scientific evidence supports the use of low-intensity shockwave therapy for the treatment of ED.  FDA has not approved shockwave therapy for treatment of ED.  As a result, shockwave is administered off-label for the treatment of ED.  The shockwave generator we use is FDA approved for treatment of other conditions but not for ED at this time.  Because shockwave therapy for ED is not FDA approved, insurance coverage for ED treatment with shockwave is limited.  Medicare does not cover shockwave therapy for ED.

How Soon Can I Expect to See the Benefit of Shockwave Therapy for ED?
