Tadalafil or Cialis in Men Undergoing Shockwave Therapy for ED: What to Expect

Will You Need to Use Cialis after Treatment with Shockwave therapy for ED?

Shockwave therapy for ED is an effective treatment option for many men.  As many as 50-80% of men experience benefit from shockwave therapy.  However, 30-50% of men either do not respond to shockwave therapy for erectile dysfunction or the benefit dissipates over time.  For men with diabetes or severe ED, maintenance shockwave therapy as administered at New York Urology Specialists may prove beneficial.

For men who use Cialis, shockwave therapy for ED can allow some men to get good erections without Cialis;  for other men, shockwave therapy may enhance the effectiveness of Cialis and lead to better erections when combined with Cialis.  

Men with hormonal problems such as low testosterone or high prolactin levels may not benefit from shockwave therapy unless the underlying problem is addressed. Treatment of low testosterone and hyperprolactinemia may prove useful.  Testosterone replacement therapy has also been proven to be beneficial in improving the effectiveness of tadalafil or Cialis in men who have low testosterone levels.

Some men with severe erectile dysfunction may not benefit from shockwave therapy sufficiently enough even when combined with Cialis, and may require penile injections with Trimix or a penile prosthesis

Men with scar tissue in the penis from injury, or calcification from dialysis-related kidney failure, may not benefit from shockwave therapy as there is not enough healthy tissue to support blood flow.  Similarly, men with fibrosis of the penis after priapism especially men with the sickle cell disease are unlikely to respond to shockwave therapy treatment.

Men with severe veno-occlusive erectile dysfunction disease or venous insufficiency which can be diagnosed by penile Doppler ultrasound are unlikely to benefit from shockwave therapy for ED as improved blood flow cannot compensate for the severity of veno-occlusive insufficiency.  Typically these men are unable to obtain sufficient erections with Trimix.  Best treatment option in this situation is an insertion of the penile prosthesis.

Is It Safe to Use Tadalafil (Cialis) in Combination with Shockwave Therapy for ED?

There are no known side effects or risks that are attributable to using Cialis during treatment with shockwave therapy for ED.  At New York Urology Specialists, we advise that men continue using Cialis at the same dose and frequency during treatment with shockwave therapy for ED.  Many men experience improved erections during treatment, and they may try to lower the dose, frequency or stop tadalafil (Cialis) to see how good their erections are without the use of Cialis.  If the erections are significantly better with Cialis, they can restart Cialis, often at a lower dose or lower frequency.
