Complex Penile Curvature (Curve Down or Up plus to the Side or Corkscrew Curvature)
Penile Curvature in Men
Complex penile curvature of the penis with a component of both ventral or dorsal curvature (pointing up or down) and a component of sideways curvature pointing to the right or left side affects approximately 20-300% of men with Peyronie’s disease.
“Ventral curvature” causes the penis to point or curve down. Dorsal curvature causes the penis to point or curve up.
A combination of lateral curvature and ventral or dorsal curvature complicates both evaluation and treatment of Peyronie’s disease. Complex penile curvature requires significant expertise in order to correctly measure the curvature. Evaluation of treatment success becomes more complicated as well.
Treatment choices are guided by the presence or absence of a palpable lump (penile plaque), the severity of symptoms, and patient preferences. If a patient experiences erectile dysfunction, the treatment of erectile dysfunction affects the choice of treatment used to treat Peyronie’s disease.

Complex Penile Curvature: Treatment Options
For men who have severe erectile dysfunction, in particular when oral treatment such as tadalafil (Cialis) or sildenafil (Viagra) do not work, or penile injections are not effective or unacceptable to the patient, a penile implant is generally an optimal treatment option which can be combined with plaque excision and/or grafting.
If Peyronie’s plaque is present, intralesional plaque injections can be used to treat ventral penile curvature caused by Peyronie’s disease. Verapamil injections, as well as interferon injections, can be used for treating ventral Peyronie’s plaque. Xiaflex has not been approved for treating ventral Peyronie’s plaques. Studies show that Xiaflex appears to be a safe option for the treatment of ventral curvature.
Penile plication can be used to correct complex penile curvature if there is no significant hourglass narrowing of the penis. Penile plication can also be used in men with a more noticeable hourglass appearance but it does not correct the hourglass defect.
Evaluation of Patients with Penile Curvature
At New York Urology Specialists, we perform an artificial penile erection with Trimix or alprostadil to get an accurate measurement of the penile curvature. Many patients with moderate to severe curvature are advised penile traction therapy in combination with penile injections (verapamil, interferon, Xiaflex). Patients with severe curvature, those who do not benefit from penile injections or interested in a quick resolution of their problems may consider surgical treatment with penile plication, plaque excision with grafting, or penile implant.
Treatment Options for Ventral Penile Curvature Include:
- Oral therapy
- Intra-lesional plaque injections (verapamil, interferon)
- Penile traction therapy
- Penile plication (16 dot plication or Nesbit plication)
- Plaque incision/excision with grafting
- Penile implant
