What is Penoscrotal Webbing? 

Penoscrotal Webbing, also known colloquially as “Turkey Neck”, refers to the skin that connects the penis to the scrotum (ball-sac bag).  When the scrotum attaches to the penis too far up the penis, it creates a skin web that may interfere with sexual function.  The closer scrotum attaches to the tip of the penis, the more severe is the penoscrotal webbing.

What are the Treatment Options for Penoscrotal Webbing Repair?


Penoscrotal Web SurgeryPenoscrotal webbing is unlikely to disappear. Typically the skin attachment is fixed and does not change.  

There are no effective creams or medications that can improve the appearance or location of penoscrotal webbing.  Stretching the skin is of dubious benefit.

Penoscrotal webbing requires surgical correction.  There are a number of surgical techniques that can be used to correct penoscrotal webbing.

The most appropriate technique depends on the exact anatomy, skin redundancy and the location of the insertion of the scrotal skin on the penis.  Z-plasty is a traditional procedure used when there is a shortage of skin but a number of additional techniques may be used depending on the particular needs of the patient, including scrotal excision and reduction, flap rotation.

If a patient with penoscrotal webbing desires a circumcision, penoscrotal repair should usually be performed simultaneously at the time of circumcision.  Many men with a pre-existing penoscrotal web that is mild, moderate or severe (without symptoms) before circumcision may develop symptoms after circumcision and would require subsequent repair if not performed at the time of circumcision. We use a special technique during circumcision to minimize the risk of penoscrotal webbing aggravation.

What are the Causes of Penoscrotal Webbing? 

Penoscrotal webbing is usually caused by a combination of factors.  Some men have a congenital (they are born that way) high insertion of the scrotal skin on the penis

When a man with a high insertion of the scrotum on the penis is circumcised, the situation is often exacerbated. Experienced urologists who perform circumcision in men with pre-existing penoscrotal web would typically modify circumcision technique to create a flap which can minimize the severity of penoscrotal web. 

Some men are advised to have penoscrotal web repair at the time of circumcision or circumcision revision.   Some men have too much skin removed during circumcision which leads to the turkey neck appearance of the peno-scrotal junction. Occasionally, penoscrotal webbing can occur as a result of scarring or fibrosis after penile or scrotal surgery such as the reconstruction of the scrotum after scrotal infection, or penile prosthesis implantation.

What are the Symptoms of Penoscrotal Webbing or “Turkey Neck” of the Penis?

  • scrotal skin riding too high on the penis
  • discomfort and irritation of scrotal skin during or after sex
  • Condoms do not stay on
  • Many men report no symptoms related to visual diagnosis of penoscrotal webbing. 

What is a “Pseudo” Peno-scrotal web?

Penoscrotal web refers to the scrotum riding high on the penis when the penis is erect and the foreskin is retracted (pulled back).  One can pull the foreskin forward over the glans penis hard enough to pull the scrotal skin along with the foreskin – this is a “pseudo” peno-scrotal web which does not cause any functional problems as it is created ‘artificially’ by pulling the foreskin over the glans penis.  “Pseudo” penoscrotal web is a frequent reason for self-referral of patients to our clinic. “Pseudo” penoscrotal web does not require surgical correction.

Why Do Men Choose to have Penoscrotal Webbing Repaired?

Depending on the severity of penoscrotal webbing and individual anatomy and sexual practices, penoscrotal webbing can cause a number of functional problems in addition to the appearance that may bother some men:

  • Difficulty rolling condoms all the way to the base of the penis
  • Condoms may slip or roll-off the penis during sex
  • Irritation of the penoscrotal web skin during sex
  • Makes penis appear shorter than it actually is
  • Makes penis functionally shorter as penoscrotal webbing can restrict the depth of penetration.

Factors that Affect Penoscrotal Webbing:

Location and severity of penoscrotal webbing affect both the severity of symptoms and the severity of aesthetic appearance. The closer the scrotum attaches to the tip of the penis or the glans of the penis, the more severe the symptoms.

The thickness of Scrotal Skin:  some men have naturally thicker and rougher scrotal skin with many ruggations.  Scrotal skin thickness exaggerates penoscrotal webbing and makes it functionally more noticeable as thick skin does not mold as easily as thinner, more pliable skin.

Men who are obese, often have a few inches of fat pad above the penis, which contributes to the short appearance of the penis and often causes buried penis.

The availability of redundant skin on the penile shaft or the scrotum can affect the surgical approach to the correction of a penoscrotal web.
