Men with diabetes are more likely to develop phimosis caused by balanitis and balanoposthitis, which is inflammation and infection of the foreskin.
The most common cause of balanitis in diabetic men is a candidal (fungal) infection. Most of the time balanitis caused by fungal infection can be treated with antifungal antibiotics. Repeated bouts of balanitis can lead to the development of fibrosis, scarring of the foreskin which leads to phimosis (difficulty with pulling the foreskin back). Small cuts or lacerations of the foreskin are common in men with recurrent fungal balanitis.
Phimosis in diabetic men tends to be more fibrotic and less responsive to corticosteroid cream. As a result, circumcision is the most effective treatment option for balanitis and phimosis in diabetic men.
