Irritation During Sex from Scrotal Skin in a 41-year old Man Treated with Penoscrotal Web Repair
JR is a 41-year-old man with complaints of irritation and discomfort in the scrotal and penile skin during sex. The problem has been present since he became sexually active but worsened with the current sexual partner.
Physical examination revealed a circumcised penis. Moderate to severe penoscrotal webbing is noted during the physical exam. The area of irritation and discomfort is located on the penoscrotal web.
The patient was advised to undergo penoscrotal web repair. He decided to have penoscrotal web repair.
The patient decided to have the procedure performed in our office under local anesthesia. He arrived at the office after having lunch earlier in the day. The patient was given prophylactic antibiotics to minimize the risk of infection and pain medications. He underwent penoscrotal web repair in our office. The procedure was done under local anesthesia and took under 90 minutes. The patient was able to go home shortly after the completion of the procedure.
On follow-up 2 weeks after the procedure, the patient reported that he had minimal discomfort and did not require any pain medications. He noticed a few drops of blood intermittently. He did not pick up a prescription that was sent to his pharmacy. He was able to return to his usual activities the next day. Examination revealed a small opening of the skin. He was advised to apply antibiotic ointment daily.
The patient was seen 3 weeks later. The scar was noted to be nearly healed He reported no pain. The resolution of the penoscrotal web was noted. The patient was advised that he can resume sexual activity in 1 week.
On follow up 3 months later the patient reported no irritation, no pain during sex. He did not notice any change in sensation, other than the resolution of the irritation and discomfort that he had prior to circumcision. He was satisfied with his decision to undergo repair of penoscrotal webbing.
