Treatment of Balanitis in Adults and Children
Balanitis refers to inflammation or irritation of the foreskin of the penis (prepuce of the penis).
What are the Causes of Penile Inflammation and Balanitis?
Balanitis can be caused by friction, chemical irritation (exposure to seawater, urine), and infection. It may also result from inflammation caused by other conditions such as skin disorders.
Infection of the foreskin can be caused by a fungus, most commonly Candida albicans (C. Albicans). Balanitis can also be caused by bacterial infections such as Staph. aureus, and others. Virals infections can also cause balanitis. Herpes is one of the most common viral causes of balanitis in men.
What is the Treatment for Balanitis in Men?
- Antibiotics (Triple antibiotic) and Antifungal Medications (1% clotrimazole cream (Lotrimin), Desitin (Zinc oxide), miconazole 2% cream)
- Corticosteroids (0.05% betamethasone cream, 1% hydrocortisone cream)
- Circumcision
The choice of treatment is guided by the most likely cause of balanitis, which can be established by an experienced urologist. For non-infectious balanitis, corticosteroids are used. For men with fungal balanitis, antifungal creams are used and a corticosteroid (anti-inflammatory) is often added. In cases where the infection appears to be bacterial, triple antibiotic cream or bacitracin ointment is used.
If topical creams do not help, circumcision offers an effective treatment option with >95% success.
Men who have severe or long-standing balanitis, balanitis accompanied by severe phimosis are unlikely to benefit from topical creams. They can choose to try topical creams or to proceed to circumcision.

Balanitis and balanoposthitis refer to the same process, namely inflammation of the penis. Balanitis refers to the inflammation of the glans penis (head of the penis); balanoposthitis refers to both inflammation of the glans penis and the foreskin. For simplicity, balanitis is often used to refer to both processes.
