What is Hypospadias and How Does it Affect Men?
Hypospadias is an abnormally placed urethral opening that is typically caused by incorrect or abnormal development of the urethra during pregnancy.
Most men who have hypospadias are born with hypospadias. Occasionally hypospadias can be caused by traumatic injury to the urethra, such as from a catheter.
For most men with mild hypospadias, the problem is cosmetic or aesthetic and does not cause any functional problems. In fact, many men are not aware that they have mild hypospadias or abnormality in the urethral opening. Many men with mild (granular or coronal hypospadias) only learn about it when they visit a urologist for an unrelated reason.
Hypospadias is classified based on the location of the urethral opening. The severity of hypospadias increases as it goes from glanular location (mild) to penoscrotal or perineal location (severe)
Hypospadias can be glanular (the urethral opening is slightly misplaced on the glans of the penis). Glanular hypospadias is the mildest form of hypospadias, and most men are not even aware that they have at. This usually does not cause any problem with urination or reproduction.
Coronal hypospadias is located on the corona of the glans penis, and it tends to be slightly more noticeable to the untrained eye. However, many men are not aware that there is a problem as they are able to urinate straight without difficulty and do not have any problems with sexual function or ejaculation. Coronal hypospadias may be associated with more noticeable cosmetic changes on the penis, and some men may desire repair of the hypospadias.
Distal penile hypospadias is located on the distal shaft of the penis and usually causes difficulty with urination standing up as well as a prominent opening of the urethra on the penis and is often associated with penile curvature (chordee).
Proximal penile hypospadias may be penoscrotal in the location on the proximal penile shaft. These are more severe types of hypospadias that typically require multistep surgical repair.
Symptoms of Hypospadias
Many mild hypospadias cause no symptoms. As the severity of the hypospadias increases, they cause more noticeable problems such as penile curvature, “Cobra-like” appearance of the penis with an open ‘urethral plate.” Men may experience problems urinating straight while standing and may have to sit down to urinate.
Men with proximal hypospadias may have problems getting a partner pregnant if the ejaculation occurs outside the vagina.
