Symptoms of Lichen Sclerosus of the Penis (BXO)?
Balanitis xerotica obliterans (BXO) of the penis, more formally known as lichen sclerosus (LS) of the penis, is an inflammatory skin condition that can affect any part of the body. At New York Urology Specialists, we focus on the treatment of lichen sclerosus of the genital area in men and women.
In particular, lichen sclerosus (also known as lichen sclerosus et atrophicus) can affect the foreskin, causing phimosis (tight foreskin) and irritation of the foreskin (balanitis). It often causes penile skin adhesion to the glans of the penis and can obliterate the corona of the penis. Lichen sclerosus is a common cause of white discoloration of the glans penis (head of the penis).
Narrowing of the urethral opening (urethral meatus) with scarring and “white” discoloration is often a symptom of LS (lichen sclerosus). This is known as meatal stenosis.

Lichen sclerosus is often a progressive disease and, if not treated early, can worsen over time. It also has a propensity for recurrence after treatment. There is an association between lichen sclerosus and penile cancer.

Lichen sclerosus is one of the most challenging skin problems to treat, especially on the penis. Anti-inflammatory medications (creams) are often the first line of treatment for mild disease. These include corticosteroid and immune modulator creams. When creams do not provide sufficient benefit, surgical treatment often results in resolution of the problem.
Surgical treatment is individualized to treat particular symptoms.
For patients with phimosis, skin adhesions, and balanitis, circumcision with excision of scar tissue is the mainstay of treatment.
Meatal stenosis is treated surgically with meatoplasty. Occasionally a tissue graft (mucal buccosal graft) may be necessary to reconstruct the urethra.
